It is proved that Qiao-shao Fang was effective and safe in premature ejaculation(PE) treatment in previous clinical practice, and the mixture can extend the ejaculation latency. It was also found in recent study that the 5-HT may have a close relationship with PE, therefore ,based on the ejaculation distribution theory, the rats will be screened and classified into four groups in this study. The intromission and ejaculation latency, intromission frequency of the rats will be observed. The content of 5-HT will be tested in the respective specimens, which were produced by separating from hypothalamus homogenate, will be detected by needle electrode insertion. To find the mechanism of the Qiao-shao Fang in inhibiting the reuptake of 5 - HT and improving the rat ejaculation times, and to provide a new theoretical guidance for clinical treatment.
课题组在前期临床实践中证实翘芍方治疗早泄安全有效,发现其能够延长早泄患者的射精潜伏期;最近研究发现,5-羟色胺可能与早泄的发生密切相关,结合最新进展,本研究将进一步探讨翘芍方治疗早泄的可能作用机制。基于连续统一体理论建立早泄大鼠模型,观察大鼠的插入潜伏期、插入次数、射精潜伏期;取大鼠下丘脑,制备下丘脑匀浆,荧光分光光度法检测各标本5-羟色胺的含量;同时观察翘芍方给予早泄患者及健康男性后外周血5-羟色胺的变化,探讨翘芍方改善射精时间治疗早泄的可能作用机制,为临床应用提供新的进一步的理论支持。
早泄是男性最常见的性功能障碍之一,严重影响患者的生活质量,为探索翘芍方治疗早泄的可能机制,课题组进行了相关研究。1.通过翘芍方治疗早泄的临床随机对照试验,证实翘芍方在延长患者射精潜伏期、提高性生活满意度、减少早泄诊断量表评分、降低中医症状评分方面与治疗前比较有显著差异,患者未见明显的不良反应安全性指标正常,证明翘芍方在治疗肝郁肾虚型原发性早泄中的安全性和有效性。2.课题组通过给大鼠每天注射5-HT1A受体激动剂(±)-8-羟基-2-(二丙基氨基)四氢萘氢溴酸盐(8-OH-DPAT)的方法,观察造模前后大鼠插入潜伏期、射精潜伏期、骑跨次数、插入次数、射精次数等性行为学指标,经验证符合早泄大鼠标准,从而成功构建了早泄大鼠模型,在早泄造模上具有重要的参考价值,是本课题的特色创新之处。3.造模成功后,课题组采用翘芍方和达泊西汀对造模成功的早泄大鼠进行干预性研究,药物干预后,再次观察大鼠性行为学指标,发现翘芍方各剂量组大鼠射精潜伏期均延长。处死大鼠,检测发现模型组下丘脑5-HT含量降低,翘芍方组5-HT含量均升高,以翘芍方高剂量组和达泊西汀组尤为明显。. 通过研究,证实翘芍方通过提高下丘脑5-HT含量,延长射精潜伏期,达到治疗早泄大鼠的目的,对中医药治疗早泄提供了理论依据,具体机制仍待进一步研究。
