The treatment based on syndrome differentiation in TCM clinic, the study on the etiological essence helps to improve clinical effect. To create a new method for etiological theory of TCM is the key scientific problems to be solved. In this study: metabolomics reflects overall state of the body which implies the bioinformation of TCM etiology(etiological essence), as the another angle for differentiating syndrome for etiology or differentiation of symptoms and signs to identify etiology. The pathogenesis of high-risk period of obese type 2 diabetes mellitus as the study object, the direct cause (thick and greasy fatness) induces Dampness-heat, using metabolomics method, and the variable importance projection (VIP) analysis techniques is introduced to solve the multiple linear problem in the biomarkers in metabolomics, obtain important biomarkers further, perform kinetic analysis, explore the transmission of etiological essence, provide the technology for objective TCM etiology. The valid prescriptions Gegenqinlian decoction(GQD) for treating Spleen Damp diabetes with clinical and experimental basis is used as a tool drug, using kinetic analytical methods, to solve the problem of the false positive biomarker occuring within the static analysis, and obtain the important biomarker in drug action, and conduct pathway analysis, to clarify the treatment mechanism of GQD on T2DM in high-risk period of bitter cold and dampness and proves the dampness-heat substantive evidence to the contrary, to provide the scientific basis of traditional Chinese medicine clinical cure disease and preventive treatment of disease.
中医临床辩证论治,病因的实质研究,有助于提高临床疗效。中医病因学研究技术方法的创建,属亟待解决的关键科学问题。本项目认为:代谢组反映机体的整体状态,其必隐含中医病因(病因实质)的生物信息,可以成为另一角度的辨证求因或审症求因。本项目以肥胖型2型糖尿病(T2DM)高危期发病过程为研究对象,以直接病因(肥甘厚腻)诱生内湿,采用代谢组学方法,引入变量重要性投影(VIP)分析技术,解决代谢组生物标记物的多重线性问题,进一步获得重要性生物标记物,并进行动力学分析,探索病因实质的传变关系,为中医病因学的客观化提供技术方法;以临床及基础实验治疗湿热蕴脾证糖尿病均有效的方药葛根芩连汤(GQD)为工具药,采用动力学的分析方法,解决静态分析可能出现假阳性生物标记物的问题,获得药物作用的重要生物标记物,并进行途径分析,阐明GQD治疗T2DM高危期的苦寒燥湿机制并反证湿热实质。为中医临床治未病提供科学依据。
中医临床辨证论治,病因实质研究,有助于提高临床疗效。通过本项目研究。阐明了GQD(葛根芩连汤)治疗T2DM高危期的苦寒燥湿机制并反证湿热实质。1.建立了湿热相关代谢组学研究方法,以肥甘厚味诱生湿热,并以葛根芩连汤进行反证。研究结果发现:①代谢组学方法可灵敏表征湿热,证明了脂代谢紊乱与IR的呈强相关。②湿热相关生物标记物显示:湿热实质主要为脂代谢紊乱,亚麻酸代谢、花生四烯酸代谢、甘油磷脂代谢为重要生物标记物代谢途径。2. 高脂饮食是肥胖、IR、T2DM的独立危险因素,60%脂肪供能高脂饲料饲养SD雄性大鼠4周可诱导肥胖产生湿热、12周可诱导形成肥胖型IR模型,28周可诱导形成肥胖型T2DM。3.肥胖型IR及T2DM大鼠血液系统不缺氧,但组织缺氧,IR组EPO(促红细胞生成素)、HIF-1α(缺氧诱导因子-1α)、Hct(血细胞比容)、ctHb(血红蛋白含量)、ctO2(血氧含量)均高于正常组 (P<0.05),且Hct、ctHb与HDL-C呈显著正相关。肥胖型T2DM大鼠,腹主动脉血液pO2、Hct、ctHb、ctO2升高(P<0.01),模型组中,Hct、ctHb、ctO2与FINS呈显著负相关,pO2与TC、HDL-C、LDL-C呈负相关。血清代谢组结果显示:长期高脂饮食或是通过影响肠道菌群和诱导慢性炎症进一步导致血脂紊乱、胰岛素敏感性降低、并最终发展为T2DM。4. GQD具有降低体重、IR并预防T2DM的作用,且能降低高脂血症及IR大鼠血清中的EPO及HIF-1α(P<0.05)。血清代谢组结果显示:GQD干预胰岛素抵抗,预防糖尿病的作用,可能是通过干预炎症反应,调节肠道菌群,改善胆汁酸代谢等机制发挥治疗预防作用。粪便代谢组显示:GQD改善高脂血症肥胖大鼠的脂代谢异常与抗炎,保护肠道屏障以及增加能量代谢等有关。本项目的研究结论可为中医临床防治糖尿病预防提供科学依据。
