The concept of the short individualized Roux-loop jejunum (SIRJ) in bilioenterostomy in child(BEC) has not been generally acknowledged in academia. That incidence of congenital biliary tract malformation is extremely low and that Sparse,scattered patients and poor long-term follow-up rate of them adverse to large-scale studies make basic and clinical research of BEC to be the devil. At present, we have established a stable animal model of BEC and conducted the thorough research on Roux jejunum for Roux-en-y cholecystoenterostomy in juvenile rabbits.Thereby we think SIRJ is the fittest Roux jejunum for Roux-en-y cholecystoenterostomy in juvenile rabbits.. The interstitial cells of Cajal (ICC) is pivotal factor of gastrointestinal movement growth and development in BEC and the latter is basis of nutrition digestion and absorption and the key of growth and development in infantile stage. It is of great theory value and clinical significance that investigation of ICC and expression of c-kit and anoctamin and their mRNA of stomach duodenum and jejunum in the scheduled juvenile rabbits and research on cellular mechanisms in biliary-intestinal anastomosis.
尽管Roux-en-y胆肠吻合术是小儿胆肠吻合术的金标准,但术中短的个体化Roux空肠肝支尚未被学术界公认。先天胆道疾病发病率极低决定了其研究的困难性。我们已建立稳定的可用于 临床和基础研究的幼兔胆肠吻合术模型并研究了Roux支,初步认为短的个体化的Roux空肠肝支更适合幼兔胆肠吻合术。目前认为胃肠道cajal细胞对于小儿胃肠道运动和生长发育起着关键作用而术后胃肠运动及生长发育是小儿营养物质消化吸收的物质基础。因而研究小儿胆肠吻合术胃肠道cajal细胞变化及机制,进而阐明胃肠运动及其生长发育的分子机制具有重要的理论价值和临床实用意义。本研究拟采用电生理,免疫组织化学、双重标记和透射电镜研究胃肠cajal细胞,应用c-kit和anotctami-1基因的mRNA表达、ckit/mSCF多个方面,探讨不同Ruox空肠肝支的小儿胆肠吻合术式对胃肠道cajal细胞的影响及分子机制。
