基于计划行为理论的医生临床路径依从行为研究

Clinic behavior of Physician determines the reasonableness of quality and cost of medical cares. Few previous studies have analyzed the subjective decision-making process of specific behavior, which usually between the impact of physician behavior patterns affect factors and the behavior output. Physician adherence to clinical pathway shows physician's respect to the medical care quality and reasonable concern to services cost. In the context of case & clinical pathway-based payment system reform in China, this research intend to study physician clinical pathway adherence behavior by theoretical and empirical analysis. The first, based on definition of physician clinical pathway adherence behavior, and survey to clinic services, expenditure, that case-based patients received and paid in six typical hospitals, to measure and describe the target behavior status and output. Second, to create a model of physician clinical pathway adherence behavior, based on theory of planned behavior and empirical analysis of decision-making process of clinical pathway adherence behavior. Finally, the study intent to elaborate the intrinsic relationship and behavior occur mechanism, among physician behavior patterns influencing factors, physician behavioral belief - attitude - intention, behavior, and the output of behavior, to reveal the key role of decision-making process in the behavior occur, and to explore the method and pathway of prediction and intervention the physician clinical pathway adherence behavior. The study based on theories and scientific evidence, intend to provide targeted policy advice to case & clinical pathway-based payment system reform and promotion of medical care quality.

医生的实际诊疗行为决定了医疗质量与费用的合理性。既往的研究很少对介于医生行为模式影响因素和行为效果产出之间的特定行为主观决策过程进行分析。临床路径依从行为反映医生对医疗质量的尊重和对医疗费用合理性的关注。 课题拟在病种临床路径支付制度改革背景下，对医生临床路径依从行为进行理论研究和实证分析。通过对医生临床路径依从行为进行界定，和6所典型医院相关病种患者所接受诊疗服务的调查，测量和描述医生临床路径依从行为现状和所产生的效果产出。基于计划行为理论和对医生临床路径依从行为决策的实证分析，建立医生临床路径依从行为模型。 研究将阐述医生行为模式影响因素、医生行为信念-态度-意图、行为发生，以及行为效果产出的内在关系和发生机制，揭示行为决策对行为发生的关键作用，探讨预测和干预医生临床路径依从行为的方法和路径。课题基于理论和科学证据，将针对性的为病种临床路径支付方式改革和医疗质量促进提供政策建议。

医生的行为已经逐步成为医改深层机制改革所关注的焦点问题之一。临床路径依从行为作为一种典型的医生职业行为，反映医生对医疗质量的尊重和对医疗费用合理性的关注程度。课题在病种临床路径支付制度改革背景下，对医生临床路径依从行为进行理论和实证分析。通过对医生临床路径依从行为进行界定，通过对相关病种患者所接受诊疗服务的调查，测量和描述医生临床路径依从行为现状和所产生的效果产出（如住院日等）。基于计划行为理论和对医生临床路径依从行为决策的实证分析，建立医生临床路径依从行为模型。研究对医生行为模式影响因素、医生行为信念-态度-意图、行为发生，以及行为效果产出的内在关系和发生机制，揭示行为决策对行为发生的关键作用，探讨预测和干预医生临床路径依从行为的方法和路径。针对性的为病种临床路径支付方式改革和医疗质量促进提供政策建议。. 成果介绍：本项目实施计划明确，实施过程基本按计划进行：2014年1月-12月，课题组完成了文献研究、设计测量工具和部分现场调查工作；2015年1月-12月，完成了现场调查、资料整理和初步分析，完成对江苏、河南、四川、新疆4省8家公立医院进行了现场调查，调查医生合计273人，分析典型病种病案1739份；2016年1月-12月，完成了补充典型现场调查、数据分析、论文撰写和成果交流等工作。课题组向国家级核心期刊和英文国际期刊积极投稿，积极参与和主办本领域的国外学术交流活动并报告、宣讲本课题的研究成果。本课题目前已完成发表相关学术论文8篇，其中已发表SCI论文3篇（其中已录用待发表1篇），国内本领域权威期刊5篇；课题研究团队成员在近年来获得自然科学基金资助立项多项。课题组计划继续通过各种渠道将研究的成果传播给行政管理和政策决策部门，并努力以国际访学的形式开展国际学术交流和成果促进。
