基于CONSORT与STRICTA指南对针刺随机对照试验报告影响性评价及其统计质量评价量表的研制与考核

The quality of reporting of randomized controlled trials (RCT) is of great significance for medical decision-making. Consolidated Standards of Reporting Trials (CONSORT) and Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) aimed at improving the quality of reporting of RCT in acupuncture treatment. However, currently there is still a lack of impact assessment research on the introduction of CONSORT and STRICTA for the reporting quality of acupuncture treatment RCT and a targeted quality rating scale. In this study, by a retrospective cohort study and prospective RCT, we will assess the impact of the introduction of CONSORT and STRICTA on the reporting quality of acupuncture treatment RCT. Based on mastering the knowledge of reliability and validity, by a standard procedure (such as the literature review, the core discussion groups, expert advice, pre-test, field test method, et al.), we will develop out the Statistical Quality Assessment Scale for RCT in acupuncture treatment which is constituted by CONSORT- common modules and STRICTA- specific modules. This scale aims at improving statistical expression level of self-examination for medical researchers and modifying level for medical editors, which is conducive to promoting the practical application of the CONSORT STRICTA in this field. At the same time, it will provide a valid and reliable methodology support for the standardization and precision of RCT report in acupuncture treatment.

随机对照临床试验（RCT）的报告质量对医疗决策具有重大意义，临床试验报告的统一标准（CONSORT）及针刺临床试验干预措施报告标准（STRICTA）旨在提高针刺RCT的报告质量，目前国内缺乏两者应用在针刺RCT报告中的影响评价及针对性的质量评价量表。本研究以回顾性队列研究及前瞻性RCT分析CONSORT与STRICTA的引入对针刺RCT报告质量的影响。在掌握信度和效度的基础上，采用国际研发量表的标准程序，如文献综述、核心讨论组、专家咨询、预试验、现场试验等方法，研制出由CONSORT的共性模块和STRICTA特异模块所构成的针刺RCT报告统计质量评价量表并考核，以便提高医学研究者的统计学表达自查及医学编辑的审阅修改水平，有利于推广CONSORT与STRICTA在此领域的实际应用。为针刺RCT报告的规范化、精确化提供有效可靠的方法学支持。

本研究以回顾性队列研究及前瞻性RCT分析CONSORT与STRICTA的引入对针刺RCT报告质量的影响。在掌握信度效度的理论基础上，采用国际研发量表的标准程序，如文献综述、核心讨论组、专家咨询、预试验、现场试验等方法，研制出由CONSORT的共性模块和STRICTA特异模块所构成的针刺RCT报告统计质量评价量表并考核，以便提高医学研究者的统计学表达自查及医学编辑的审阅修改水平，有利于推广CONSORT与STRICTA在此领域的实际应用。为针刺RCT报告的规范化、精确化提供有效可靠的方法学支持。. 通过项目的前期研究发现：在CONSORT和STRICTA出版后3-4年，针刺RCT的报告质量较出版前有明显提高（P≤0.05）；有研究表明，CONSORT和STRICTA将有利于规范针刺RCT的报告质量，提高研究透明度。在课题第二部分研究方面，将两个指南结合，通过文献综述和专家访谈等方法设计了《针刺随机对照试验研究报告的统计质量评价量表调查问卷》，根据收集回来的评价意见筛选条目池，进一步确定第一阶段的条目池，形成量表初表，并进一步评价量表的信度和效度，形成《针刺随机对照试验报告统计质量评价量表》，运用该评价量表可对论文的报告质量进行评分，通过确定临界值分数，让论文作者及编辑对论文的水平进行核查，并可以看出哪一方面做得不够。为了更好地区分论文是否达到SCI期刊的发表水平，运用ROC曲线确定SCI论文统计质量评分的临界值。根据计算的结果提示，运用随机对照试验报告统计质量评价量表对论文评价后的得分如果在51.5分或以上，则达到SCI论文的报告质量要求。. 课题已按计划进度要求完成所有的课题研究工作，并已完成“针刺随机对照试验报告统计质量评价量表”的研制，公开发表相关学术论文两篇，没有存在研究进程延后、研究方案变更、人员变更、经费使用延后等情况。
