Immune thrombocytopenic purpura (ITP) is an immune-mediated thrombocytopenia syndrome and belongs to "blood syndrome" of Traditional Chinese Medicine(TCM),however the conventional treatment is poor for most ITP-patients, who are mostly belonging to "spleen failing to manage blood" syndrome of TCM.Chinese medicine treatment for ITP is effective and stability, rebound phenomenon rare. Based on research about PD-1/PD-L1 signaling pathway and Treg/Th17 cells in immune regulation and autoimmune disease, suggesting which may play important role in ITP pathogenesis. In this study,we base on "spleen failing to manage blood" theory of TCM and modern immune mechanisms, intervene in mouse model of spleen failing to manage blood with Guipi granules,then use technology as ELISA, FCM, RT-PCR and immunohistochemistry to explore effect mechanisms of Guipi granules to treat ITP from different levels including platelet count, megakaryocyte cell morphology, platelet-associated antibody, PD-1/PD-L1,Treg/Th17 cell function and functionally related cytokines, Foxp3mRNA, RORγtmRNA expression levels and so on, attempt to clarify the pathogenesis of ITP of "spleen failing to manage blood" and expand the research areas of ITP pathogenesis, further to provide theoretical and experimental basis for the Chinese medicine treatment of ITP.
免疫性血小板减少症(ITP)是一种免疫介导的血小板减少综合征,属中医"血证"范畴,多数患者常规治疗效果不佳,这部分患者多属于中医的脾不统血。中医药治疗ITP疗效稳定,"反跳"现象少。基于PD-1/PD-L1信号通路及Treg/Th17细胞在免疫调节及其在自身免疫性疾病方面的研究成果,推测其在ITP发病中可能发挥着重要作用。本研究以中医"脾不统血"脏腑气血理论与现代免疫机制结合,建立ITP动物模型,用归脾冲剂进行干预,利用ELISA、FCM、RT-PCR、免疫组化等技术,从血小板计数、骨髓巨核细胞形态、血小板相关抗体、PD-1/PD-L1、Treg/Th17细胞功能及功能相关的细胞因子、Foxp3mRNA、RORγt mRNA表达水平等不同层探索归脾冲剂治疗ITP的效应机制,试图阐明ITP脾不统血的病机本质,拓展ITP发病机制的研究领域,为中医治疗ITP提供理论和实验依。
免疫性血小板减少症(ITP)是一种免疫介导的血小板减少综合征,属中医“血证”范畴,多数患者常规治疗效果不佳,这部分患者多属于中医的脾不统血。中医药治疗ITP疗效稳定,“反跳”现象少。基于PD-1/PD-L1信号通路及Treg/Th17 细胞在免疫调节及其在自身免疫性疾病方面的研究成果,推测其在ITP发病中可能发挥着重要作用。本研究以中医“脾不统血”脏腑气血理论与现代免疫机制结合,建立ITP 动物模型,用归脾冲剂进行干预,利用ELISA、FCM、免疫组化等技术,从血小板计数、骨髓巨核细胞形态、血小板相关抗体、PD-1/PD-L1、Treg/Th17 细胞及其相关细胞因子(IL-10、IL-17、IL-23、IL-21、TGF-β1)表达水平及非经典免疫指标SIgA(分泌型免疫球蛋白A)、β-EP(β-内啡肽)、VIP(血管活性肠肽)等不同层面探索归脾冲剂治疗ITP 的效应机制,试图阐明ITP 脾不统血的病机本质,拓展ITP发病机制的研究领域,为中医治疗ITP 提供理论和实验依。. 本研究成功制备了豚鼠抗Balb/C小鼠血抗小板血清(GP-SPS),注射APS免疫造模法能够导致小鼠外周血小板减少,注射APS免疫造模法的ITP小鼠骨髓巨核细胞及其分类符合人类ITP骨髓象特征,观察ITP小鼠一般情况(活动状态、疲劳情况、体重、体温、饮食饮水等),其病症符合中医脾气虚(脾不统血证)证侯特点,免疫法能够成功复制ITP动物模型。.本研究结果提示归脾冲剂对ITP有较好的治疗作用,归脾冲剂能有效恢复ITP小鼠的血小板计数,并能促进骨髓巨核细胞向成熟方向分化,与强的松疗效相当。ITP存在免疫功能紊乱情况,表现为IL-10、Treg细胞、SIgA、VIP表达降低,IL-17、IL-23、IL-21、TGF-β1、Th17细胞、β-EP升高,脾脏PD-1表达增高,PD-L1表达降低,其发病可能Treg/Th17细胞及其相关细胞因子失衡、非经典免疫紊乱、PD-1/PD-L1表达异常相关,归脾冲剂可能通过调节Treg/Th17细胞及其相关细胞因子平衡,改善非经典免疫指标,促进Treg细胞增殖及TGF-β1分泌,并能调节脾脏PD-1/PD-L1的异常表达,进而调节ITP免疫异常,降低血小板相关抗体,减少血小板破坏,促进骨髓巨核细胞向成熟方向分化而达到治疗效果。
