Diabetic microvascular complication is a complex disease affected by both genetic factors and glucose status. As the phenotypes of complications are complex and difficult to collect, the genetics of diabetic microvascular complication is a scientific question remained to be answered. Previously we performed several epidemic studies including Shanghai Diabetic Complications Study and established the database for diabetic chronic complication studies in Chinese. In this project, we plan to investigate the association between diabetic microvascular complications and ~40 loci linked to beta cell function in ~4000 subjects from these epidemic studies, the loci showed association to disease will be replicated in additional ~6000 subjects recruited from Hong Kong Diabetes Registy. We will also carry out functional studies to elucidate how the variant(s) participate in the disease. This project is important for the understanding of genetic mechanism of type 2 diabetes in the Chinese and will promote the genetic study on diabetic microvascular complications in the Chinese.
糖尿病微血管病变是一个受到血糖控制和遗传因素共同影响的复杂疾病。由于其临床性状复杂,样本难以收集,一直是遗传学研究的难点。课题组前期开展了Shanghai Diabetic Complications Study等流行病学研究,已建立了用于中国人2型糖尿病微血管病变的研究样本,本课题拟以此人群为研究对象(筛选后~4000人用于本研究),选取与胰岛功能相关的40余个基因的SNP位点,开展遗传关联研究,寻找与2型糖尿病微血管病变相关的易感位点,并利用香港合作人群进一步验证(~6000例),同时进一步分析各位点对患病程度的影响。经确认的易感位点,将初步开展功能实验,为阐明疾病的分子机制奠定基础。本研究对认识中国人2型糖尿病肾病和眼病并发症的遗传机制具有重要理论意义,将推动2型糖尿病微血管并发症的病因学研究。
糖尿病微血管病变是糖尿病重要的慢性并发症,其发病机制尚未明确。除了血糖控制、糖尿病病程影响其发生外,遗传因素也参与了其发生发展。但由于其临床性状复杂,样本难以收集,针对糖尿病微血管病变的遗传学研究进展比较缓慢。本项目组在~4,000例具备精细临床表型的上海地区样本中开展了影响胰岛β细胞功能的基因的常见变异与糖尿病慢性并发症的关联研究,研究共纳入85个2型糖尿病易感位点。结果发现,在校正常见混杂因素后ADAMTS9-AS2 rs4607103、CDKAL1 rs7756992和IDE-KIF11-HHEX rs1111875与糖尿病眼病显著相关;TCF2 rs4430796与糖尿病肾病显著相关,ZFAND6 rs11634397与肾小球滤过率(eGFR)相关。此外,项目组对国外全基因组关联报道的糖尿病眼病的易感位点在中国2型糖尿病人群中进行了验证,发现ZNRF1 rs17684886 及COLEC12 rs599019与糖尿病眼病及其严重程度相关,SCYL1BP1 rs6427247与API5 rs899036与重度糖尿病眼病相关。随后,开展的炎症相关基因的常见变异与糖尿病微血管并发症的关联研究中发现CRP常见变异与糖尿病眼病发病风险相关,而TLR4常见变异与糖尿病肾病无关。通过这些研究,发现了参与中国人2型糖尿病微血管病变发生发展的新遗传标记,为其发病机制研究提供重要的新认识,并有望为易感人群的筛查及早期防治创造条件。此外,项目组研究发现在2型糖尿病患者中,随着血尿酸水平增高,糖尿病肾病的患病风险增高,血尿酸水平是糖尿病肾病的独立危险因素;主动、被动吸烟均为2型糖尿病患者发生心血管疾病的危险因素。本项目累计发表SCI论文12篇,影响因子大于5分1篇,累计影响因子32.73。协助培养硕博士研究生4名并多次参加国内外合作交流。
