In our country, 150,000 to 200,000 new cases of ONFH (osteonecrosis of femoral head) were dignosised each year. Blood supply obstacles lead to osteonecrosis of femoral head and secondary collapse is lead to hip osteoarthritis secondary to the main cause of pain and disability. Therefore, the treatment of femoral head necrosis must have the combination of osteogenesis,chondrogenesis and vascularization. The project will make use of recombinant adenovirus hepatocyte growth factor infection tendon derived stem cells, and the combination of hepatocyte growth factor to promote vascularization and apoptosis resistance function of osteogenesis, of tendon derived stem cells differentiated into cartilage and bone, by means of cell therapy, bone repair, will put forward a new idea for treatment of femoral head necrosis. Transplant related to cell survival is the key factor that decides the success or failure of the cell therapy. Hepatocyte growth factor is a kind of resistance to apoptosis cells can be applied to many cells, transplantation of local high levels of hepatocyte growth factor can improve the tendon stem cell survival rate, chemotaxis injury surrounding stem cells, improve the bone healing process. Therefore, the research will also provide a beneficial reference for other cell therapy. This topic based on biomechanics and tissue engineering, in support of weak solution to femoral head necrosis area at the same time, to promote bone repair osteonecrosis area reconstruction, so as to prevent the collapse of femoral head, delay the artificial joint replacement.
我国每年新发股骨头坏死病例15-20多万例,股骨头血液供应障碍导致骨坏死,继发头塌陷是导致髋关节骨性关节炎继发疼痛、致残的主要原因。因此,治疗股骨头坏死必须将促成骨、成软骨和促血管化有机结合。课题将利用重组肝细胞生长因子腺病毒感染肌腱干细胞,结合肝细胞生长因子的促血管化和抗细胞凋亡以及肌腱干细胞的成骨、成软骨功能,通过细胞治疗的手段,进行骨坏死修复,将为治疗股骨头坏死提出一个新的思路。移植相关的细胞存活是决定细胞治疗成败的关键因素。肝细胞生长因子是一种可作用于多种细胞的抗凋亡细胞因子,移植局部高浓度的肝细胞生长因子可提高肌腱干细胞存活率,趋化损伤部位周围干细胞,改善骨修复过程。因此,课题研究也将为其他细胞治疗提供有益的参考。本课题依托生物力学及组织工程,在解决股骨头坏死区域支撑薄弱的同时,促进骨坏死区域骨修复重建,从而预防股骨头的塌陷,延缓人工关节置换。
