The current drug eluting stents have delayed stent endothelialization and increase late coronary stent thrombosis risk. Ginkgo first contained in the" BENCAOPINHUI", with promoting blood circulation to remove blood stasis, broad chest pain relief role, the main active ingredient is lactone compounds. Ginkgolide B is its main active substance. Previous studies We found that the effects of ginkgolide B can inhibition effect of rapamycin on inhibition of endothelial cell function by down regulation of P38MAPK signal pathway, our in vitro experiments clearly ginkgolide B compound drug stent has good stent drug release curve, and in a week of porcine coronary artery balloon injury in the experiment proven to promote the role of early stent endothelialization. This study will further clarify the effects of ginkgolide B on vascular endothelial cell growth promoting effect, and in vitro p38MAPK signal pathway in vitro. We will ginkgolide B compound drug eluting stent, proposed by arteriovenous shunt thrombosis test and high load test study of ginkgolide B compound drug eluting stent thrombosis, coronary artery balloon injury in porcine experimental evaluation of ginkgolide B compound drug eluting stents in porcine coronary arterial intimal proliferation and endothelialization and the support of local endothelial effect on p38MAPK signaling pathway. The purpose of is to clear the ginkgolide B compound drug stent inhibition of neointimal proliferation and promoting early endothelialization and antithrombotic effect and effect on p38MAPK signaling pathway.
目前药物洗脱支架有延迟支架内皮化及增加晚期冠脉支架内血栓的危险。银杏首载于《本草品汇精要 》 ,其具有活血化瘀,宽胸止痛的作用,其主要活性成份是内酯类化合物。银杏内酯B是其主要活性物质。 前期研究我们发现银杏内酯B可通过下调p38MAPK抑制雷帕霉素对内皮细胞抑制的作用,我们在体外实验中明确银杏内酯B复合药物支架具有良好的支架药物释放曲线,并在一周的猪冠脉球囊损伤实验中证实其具有早期促进支架内皮化的作用。本研究将制备银杏内酯B/雷帕霉素复合药物支架,并检测其释放及降解等物化性质,并通过体外细胞及血液及动静脉分流试验和高负荷血栓试验研究其抗血栓及促内皮化作用,通过猪冠状动脉球囊损伤试验评价该药物支架对猪冠脉内膜增殖与内皮化的作用及对支架局部内皮p38MAPK信号通路的影响。 本研究的目的是明确银杏内酯B/雷帕霉素复合药物支架对猪冠脉早期内皮化及抗血栓的作用和对p38信号通路产生的影响。
药物洗脱支架可显著降低冠脉介入治疗再狭窄率的发生,但是支架内皮化不良、支架晚期血栓形成是经皮冠状动脉介入术的突出问题 。近年研究表明,药物洗脱支架植入后可出现支架内血栓、聚合物高敏反应、支架贴壁不良、动脉瘤等并发症,而支架内血栓尤其是晚期支架内血栓是药物洗脱支架(DES)最严重的并发症,已引起国内外学者的高度重视。课题组拟将银杏内脂B与雷帕霉素联合应用于冠脉药物支架系统,希望在保留其抗再狭窄作用的同时,研究出一种同时具备促进内皮化及抗血栓作用的复合药物洗脱支架。.本研究通过制备银杏内酯B/雷帕霉素复合药物支架并检测其物化性质;研究银杏内酯B/雷帕霉素复合药物支架对血管内皮细胞和血管平滑肌细胞生物学特性及p38MAPK信号通路调控的作用;研究银杏内酯B/雷帕霉素复合药物支架对血液的相容性影响;研究银杏内酯B/雷帕霉素复合药物支架对猪冠脉球囊损伤的实验等四方面研究对银杏内酯B/雷帕霉素复合药物支架对冠脉促进早期内皮化和抗血栓的作用。.研究结果银杏内酯B/雷帕霉素复合药物洗脱支架具有良好稳定的物化性质;聚合物聚乳酸在7天降解率达到90%;银杏内酯B和雷帕霉素在28天释放率达到85%;银杏内酯B能够通过p38MAPK信号通路途径抑制雷帕霉素对内皮细胞的损伤;血液相容性研究提示银杏内酯B/雷帕霉素支架有强大的抗血栓作用,未出现溶血等不良反应;在大动物实验中提示,银杏内酯B/雷帕霉素具有良好促内皮化、抗血栓的作用,具有冠脉支架抗再狭窄的作用。.银杏内酯B/雷帕霉素复合药物洗脱支架具有良好支架生物相容性,具有早期促进支架内皮化及良好的抗血栓作用。
