Lumbar adjustment manipulation is the core therapy to Lower back pain in Traditional Chinese Medicine, which has complicated operation principle and difficult operation, so it is a needed problem of researching for operation principle with Lumbar adjustment manipulation. Research team made previous simulated studies of operation principle with lumbar oscillation-adjustment manipulation, And found that Compared to the traditional force mode of "sprain force" and "frustrated force", which is characterized by "Impulse", "oscillatory force" force mode could make a "resonance" effect, and has been saving, safe and good clinical efficacy, etc. But it still needed an objective, quantitative research for resonance theory of lumbar oscillation-adjustment manipulation in manipulation feature and mechanical loads. The study will make a vivo and in vitro test, which is to collect multivariate data in kinematics, dynamics and force transmission effects of lumbar oscillation-adjustment manipulation and traditional lumbar adjustment manipulation, to establish lumbar adjustment manipulation mechanics database, by establishing a multi-lever lumbar biomechanical model, to analysis lumbar oscillation-adjustment manipulation in manipulation feature and mechanical loads, and optimal combination of mechanical characteristics of diverse elements of frequency, amplitude, acceleration, etc, under "resonance" effect. Then to investigate the optimized mechanism of the mechanical loads with lumbar oscillation-adjustment manipulation, providing an objective evidence for security and popularization of manipulation.
腰椎调整手法是中医推拿治疗腰椎病的核心手法,手法动作原理复杂,操作难度大,对腰椎调整手法操作原理的客观、量化研究是目前亟需解决的难题。课题组前期对腰椎振荡调整手法动作原理进行模拟分析,发现相较于传统“闪挫力”、“顿挫力”等以“脉冲力”为特征的施力方式,该手法“振荡力”为特征的施力方式,可使患者产生“共振”效应,具有省力,安全性高等优点,并取得良好疗效,但尚需对该手法动作特征及力学加载的“共振”原理进行客观、量化研究。本课题拟通过在体、离体试验,实时采集腰椎振荡调整手法与传统腰椎调整手法运动学、动力学及力传递效应等多元力学数据,建立腰椎调整手法力学数据库,通过多杠杆腰椎生物力学模型,分析腰椎振荡调整手法动作及力学加载模式的特征,以及产生“共振”效应时,频率、振幅、加速度等多元力学要素的优化组合特征,探讨腰椎振荡调整手法力学加载模式的优化机制,为手法安全应用及推广提供客观依据。
课题组按照任务方案,开展了振荡腰椎调整手法与传统腰椎调整手法运动学、动力学及力传递效应等在体及离体研究,实现了预定目标。.首先,开展了腰椎调整手法关节“咔哒”声声学定位算法研究并制备腰椎“咔哒”声声学检测装置,完成了腰椎调整手法关节“咔哒”声声学效应研究。.其次,对振荡腰椎调整手法及传统腰椎斜扳法两阶段交叉随机对照研究,发现:1.所有瞬间加载的力,如“闪挫力”都属于脉冲式施力;2.发现两种腰椎斜扳法的脉冲力和振荡力加载周期特征,且振荡力的频率并不固定;3.单次振荡式腰椎调整手法操作的最大加载力小于传统腰椎斜扳法,其准确率高于传统腰椎斜扳法,且手法完成时对腰椎其它椎体节段的影响小;4.振荡腰椎调整手法对腰部竖脊肌硬度差值改善更优。.第三,建立腰椎调整手法多元要素杠杆力效应分析模型,对两种调整手法运动学和动力学原理进行数学模拟分析。研究发现:1.传统腰椎斜扳法的“闪挫力”为脉冲式加载方式,在0.125秒至0.25秒(频率4Hz~8Hz)内完成;振荡式加载,连续振荡操作的峰值间频率大于4Hz会因与脊柱固有频率不符,而导致关节振幅减小,且频率并不固定;2.单次振荡式腰椎调整手法操作的最大加载力小于传统腰椎斜扳法,其准确率高于传统腰椎斜扳法,且对腰椎其它椎体节段的影响小;3.振荡腰椎调整手法对腰部竖脊肌硬度差值改善更优,且更安全;4.对腰椎系统而言,其弹性特性对振荡手法的作用效果影响较大,为系统的核心要素,其阻尼特性对振荡手法的作用效果影响不大,并非系统的核心要素。5.对振荡推拿手法而言,其操作频率越高,作用效果越显著,同时,随着振荡次数的增加,手法作用效果也会增加但不会无限增加,而是存在一个上限。.综上,课题组建立一种新的分析腰椎调整手法杠杆力加载原理的研究方法,阐释振荡式腰椎调整手法杠杆力多元要素加载的优化机制,对关节调整手法提出了新的分类标准及临床理论依据,拓宽了相关生物力学基础研究的领域及思路,进一步完善推拿手法核心技术体系。
