Skin and skin structure fungal infections (SSSFI) are an important disease affecting health of individuals with protracted course, or progress to life-threatening. Many clinical phenotypes of SSSFI, such as cellulites, have not been fully characterized, mainly due to difficulties in the determination of pathogens. Pathogens of SSSFI are conventionally thought to be limited in dermatophytes, while deep infections are only associated with Candida spp. and other known species. Many opportunistic fungi cannot be successfully isolated, or are often been considered as contaminating fungi even recovered. Recently, we have realized that the clinical phenotypes of SSSFI are far more than that have been recognized traditionally, with a much wider range of pathogenic fungi species. It is therefore of great importance to understand the SSSFI clinical phenotypes, pathogenic fungal diversity and possible associations. This project is aim to conduct the study on pathogenic fungal diversity of clinical fungal isolates from SSSI in our hospital with morphologic, phyciologic and phylogenetic analysis;. Clinical research intended to histopathologic methods, fluorescence in situ hybridization method using confocal laser scanning microscopy, transmission electron microscopic observation of micro-morphological characteristics of fungi in tissues. With clustering analysis methods, the correlations between infection type and pathogenic species will be discussed. The results would improve the awareness of the SSSFI diversity of pathogenic fungi, and discovery of new species and novel Chinese record species. It would also provided important reference for scientific identification of pathogenic fungi, that are important for diagnosis and treatment of SSSFI in China.
皮肤及皮肤结构真菌感染(SSSFI)是影响国民健康的重要疾病,常迁延不愈,甚至威胁生命。目前很多临床表型未被充分认识。一方面原因是对致病真菌认识不清、分离困难和鉴定不准确;另一方面是临床表型及物种多样性研究不全面.传统观念认为浅部SSSFI致病菌多为皮肤癣菌,深部为念珠菌等已知属种。经典分离方法造成许多真菌不能成功分离,或被鉴定为污染菌。申请者多年来的研究显示SSSFI临床表型远超过传统认识;其致病真菌属种类繁多。本项目拟对SSSFI临床分离菌株进行属种多样性研究,包括形态学、生理学及分子系统学;对非经典属种及非经典临床类型进行真菌病理学研究,包括组织特殊染色、FISH激光共聚焦显微镜定位、透射电镜观察超微形态;对致病属种和临床表型进行相关性研究。研究结果将全面提高对SSSFI致病真菌多样性的认识,并发现新种和中国新记录种,也将为中国SSSFI病原真菌的科学鉴定提供重要理论依据。
