皮瓣是重大创伤和整形外科重要的修复手段,然皮瓣坏死屡见不鲜,尽快建立血液循环是皮瓣成活的关键。VEGF-Dll4/Notch信号通路在血管再生中发挥着重要作用,成为近年来研究的热点,但在促进皮瓣血管再生方面却未见此类报道。促进血管再生,临床多采用血管生长因子等外源性有创手段。近年来,中医药在这方面的取得了一定的研究进展。前期研究发现,消肿止痛合剂能促进鼠尾再植后的血管修复,提高VEGF表达水平。本课题在此基础上,提出假设:消肿止痛合剂能够使VEGF-Dll4/Notch信号通路在血管再生过程中精确统一表达,能够产生数量多且质量好的血管。本研究借助动物模型、细胞模型,以消肿止痛合剂干预,通过观测大鼠模型皮瓣新生血管形态学、体外细胞培养分子生物学等指标,后采用特异性阻断剂,观察消肿止痛合剂对阻断通路后的影响,探讨消肿止痛合剂对皮瓣血管再生的作用机制及靶点,旨在为皮瓣成活提供一种新思路和新方法。
