睡眠-觉醒昼夜节律紊乱所带来的社会、健康问题日益突出,国内外学者正在挖掘针刺作为其潜在治疗手段。中医学认为营卫之气的正常循行是维持机体昼夜节律的基础,阴阳跷脉受营卫之气影响而调节睡眠。课题组前期运用针刺阴阳跷脉获得了良好临床疗效,并发现其作用与调节“褪黑素(MT)”相关。cAMP-PKA-CREB通路是睡眠-觉醒昼夜节律二级调控及MT合成的关键环节,综上,本次研究提出以下工作假说:针刺信号→激活cAMP-PKA-CREB信号通路→MT→调节睡眠-觉醒昼夜节律。. 为验证该假说,本次选取MT分泌昼夜节律明显的金黄地鼠为研究对象,进行针刺干预,并利用高自动化的睡眠生物解析系统与时间生物学的分析方法,从MT介导的cAMP-PKA-CREB通路在松果体中的昼夜节律变化规律进行研究,以期阐释针刺调节昼夜节律紊乱的分子机制,丰富中医时间医学的科学内涵,为针刺治疗CRSD提供理论依据。
近年来,随着电子产品使用、跨时区飞行、倒班从业人员等的增多,睡眠-觉醒昼夜节律紊乱所带来的社会及健康问题日益凸显。中医学认为营卫之气的正常循行是维持机体昼夜节律的基础,阴阳跷脉受营卫之气影响而调节睡眠。课题组前期运用针刺阴阳跷脉获得了良好临床疗效,同时,发现针刺可能通过“褪黑素(MT)”发挥其生物调节效应。cAMP-PKA-CREB通路是睡眠-觉醒昼夜节律二级调控及MT合成的关键环节,本次研究旨在进一步挖掘针刺在昼夜节律二级调控中的作用,并初步探讨针刺在昼夜节律一级调控与二级调控衔接过程中可能发生的作用。提出以下工作假说:针刺信号→→激活cAMP-PKA-CREB信号通路→→MT→→调节睡眠-觉醒昼夜节律。本次选取MT分泌昼夜节律明显的金黄地鼠为研究对象,开展模型鉴定、效应观察及机制探讨三大部分研究内容。采用高度自动化的睡眠生物解析系统连续记录金黄地鼠24小时脑电图,并通过现代时间生物学方法客观评价针刺对睡眠-觉醒量及昼夜节律的影响,发现:针刺组Wake、REM、NREM昼夜节律恢复,振幅趋向于正常;与针刺+NS组相比,针刺+H89组的8h Dark REM、24h Total NREM 、16h Light NREM、8h Dark NREM均显著降低,针刺“申脉、照海”对模型地鼠Wake、REM昼夜节律以及NREM睡眠量的调节效应可被PKA抑制剂H89阻断。此外,采取多节点采样的动态节律分析方法,对MT、其合成限速关键酶AANAT、HIOMT及上游通路cAMP-PKA-CREB在外周及中枢的表达变化进行研究,发现:针刺可上调黑暗阶段松果体中PKA、CREB、AANATmRNA表达,调节PKA表达的昼夜节律,促进外周AANAT、MT分泌,发挥其对睡眠-觉醒昼夜节律的调节效应,该效应可被PKA抑制剂H89阻断。初步证实了cAMP-PKA-CREB信号通路是针刺调节睡眠-觉醒昼夜节律紊乱的作用机制之一,丰富了中医时间医学的科学内涵,为针刺治疗昼夜节律紊乱型睡眠障碍提供了理论依据。
