胃癌的病因尚未完全阐明,实施针对病因的一级预防比较困难。胃癌前病变(PLGC)在胃癌发生中起到了重要作用,因此对胃癌前病变进行及时有效的处理将是预防胃癌的关键。我们经过近20余年的临床和实验研究总结PLGC的病机关键是气阴两虚挟瘀挟湿,以益气养阴化瘀解毒法为大法创立经验方欣胃颗粒用于胃癌前病变的防治取得了显著的疗效。PI3K/Akt是与细胞增殖和凋亡关系最密切的信号转导通路之一,并通过对P-糖蛋白(P-gp)的调控影响胃癌的多药耐药从而对胃癌的治疗起到了重要作用。本项研究拟在PI3K/Akt通路对P-gp的调控平台上,研究欣胃颗粒对PI3K/Akt信号通路的影响,以及对下游P-gp表达的调控,揭示其对胃癌前病变的防治作用和机理,为中医药参与胃癌前病变的综合防治以及逆转胃癌多药耐药提供理论依据。
