Disease of the intervertebral disc is a serious disease that endangers the health of human and animal. As a representative of modern acupuncture technique, electroacupuncture has demonstrated a good effect of regulating Qi and blood circulation on the canine paralysis caused by extruded intervertebral disc oppressing the spinal cord. Previous studies indicated that the oppression on spinal cord by extruded disc resulted in decreased blood perfusion in microcirculation and number of blood vessels, and other secondary damages such as degeneration and necrosis of neuron cells, which caused the dysfunction of nerve conduction and paralysis of animals. However, electroacupuncture had displayed a significant improvement for this disorder. But how the electroacupuncture improve blood perfusion in microcirculation and number of blood vessels then promote nerve regeneration and function recovery of spinal cord in short time need further research. Effects of acupuncture has close relationship with microcirculatory blood flow and endothelial cell. This project will study the effect of electroacupuncture on endothelial vasomotor factors, growth factors, adhesion and anti-inflammatory factors of spinal cord capillary of "Wei Syndrome" dog, reveal the molecular mechanism of electroacupuncture on the microcirculation of the spinal cord function, at the same time, analyze the signal transmission pathway from the periphery to the lesion site of spinal cord using evoked potential technique, and accumulate data for revealing the mechanism of acupuncture action.
椎间盘病是危害人类和动物健康的重要疾病。电针作为现代针灸的代表技术,对犬椎间盘脱出压迫脊髓引发的瘫痪显示出了良好的理气活血效应。前期研究表明,椎间盘脱出压迫脊髓,引起脊髓微循环血流灌注量下降、血管数量减少,进而表现神经细胞变性、坏死等继发性损伤是导致神经传导障碍和动物瘫痪的关键病理变化,对此电针具有显著的改善作用。电针是如何在短时间内改善脊髓微循环血流灌注量,增加微血管数量,减轻损伤并促进神经的再生和恢复脊髓的功能,从而发挥其理气活血效应的机理需要进一步研究。针灸效应,离不开针刺信号对微循环血流量的调节和对内皮细胞功能的影响。本项目将在研究电针对痿证犬脊髓微血管内皮细胞舒缩因子、生长因子、黏附因子和抗炎因子的影响,揭示其对脊髓微循环作用的分子机制的同时,运用诱发电位技术分析其信号从末梢到脊髓病变部位的传递路径及进而改善微循环血流、恢复脊髓功能的微观机制,为揭示针灸作用的机理积累资料。
电针对犬椎间盘脱出引发的瘫痪显示出了良好的临床疗效。前期研究显示电针作用与改善脊髓微循环密切相关,本研究通过犬和大鼠模型,使用影像学、激光多普勒、分子生物学和诱发电位等技术,进一步揭示了脊髓损伤和电针促进修复的相关机制。.研究结果显示:1.脊髓腹侧和背侧气囊压迫法可成功制作犬椎盘脱出模型,用于相关实验研究;2.电针临床治愈脊髓持续压迫犬的疗效,证明解除脊髓压迫并非治疗椎间盘脱出导致瘫痪的必须条件;3.椎间盘脱出后,患部血流灌注量下降,而电针能够调节血管舒张因子NO、血管收缩因子内皮素(ET-1)和与微血管再生相关的血管内皮细胞生长因子(VEGF),从而增加微血管数量和血流量;4.脊髓受到压迫后发生神经细胞坏死等病理变化,电针能够减少神经细胞坏死,从而保持和恢复脊髓形态学和功能的正常;5.电针能够抑制致炎因子NF-α和IL-6,促进抗炎因子IL-10的产生,从而通过调控炎症反应实现治疗作用;6.电针能够激活脊髓神经干细胞,诱导干细胞向神经细胞分化,增加神经营养因子(NGF)、脑源性神经生长因子( BDNF)、神经营养素-3(NT-3)(NGF)的产生,促进受损的神经元再生;7.电针可以降低半椎板切除犬血浆中四种致痛物质的含量;8.脊髓受到压迫损伤时,体感诱发电位(SEP)潜伏期延长、波幅下降,电针治疗后显著恢复;9.针刺末梢穴位对脊髓损伤的疗效和增强刺激信号的传导,并提高动物运动机能的作用,与末梢神经功能的完整性相关;10.电针可以通过调节基质金属蛋白酶、解聚样金属蛋白酶及抑制因子的表达,来减缓细胞外基质的降解,维持细胞外基质的代谢平衡,从而发挥防治椎间盘退变的作用。.电针治疗椎间盘脱出的主要机理是:刺激末梢穴位,增强神经传导性,提高脊髓对压迫的顺应性,抑制炎性因子、致痛物质、促进抗炎因子释放,增加微血管数量,改善微循环,激活、诱导神经干细胞分化,促进神经元再生,保持脊髓形态学和功能的正常发挥治疗作用,并通过抑制椎间盘退变对椎间盘病发挥预防作用。
