基于共享决策的老年人社区健康服务优化与助推策略研究

How to promote the healthy aging has become an issue of the society in China. Not only should it meet the primary health demands of the elderly, but also meet their growing demands for a better life. From these perspectives, this project concentrates on the mismatch matter of supply and demand in health service for the elderly, concerning about the community health service for the elderly, and focusing on adapting and changing the usage behavior pattern of health service for the elderly. As a result, based on the shared decision-making idea, a scientific preference analysis tool is developed to put forward the strategy of promoting the supply of community health service for the elderly, which follows the train of thought of demand-oriented, service accessibility and quality improvement. On one thing, in view of evidence-based ideas, the community health service items which are demanded by the elderly are investigated from the perspective of holistic health. On the other thing, based on the analysis of preference characteristic, an analytic framework of discrete choice experiments is constructed considered individual heterogeneity of the elderly, using discrete choice experiments with random-parameters (mixed) logit and latent class model, to deeply explore the elderly demand for the community health service items. The last but most important thing is to put forward an optimization and nudge strategy of community health service for the elderly through the investigation of both rational and irrational preferences of the elderly, under the analytic framework of discrete choice experiments. Furthermore, this project aims to provide a decision-making basis for the government. It is not only to help the optimal allocation of primary medical and health resources, but also to reflect the spirit of humane care in the context of Comprehensive Health.

如何促进健康老龄化已成为全社会关注的焦点，其不仅要满足老年人的基本健康需要，更要满足老年人日益增长的美好生活需要。本项目针对老年人健康服务中的供需错配问题，以老年人社区健康服务为研究重点，以适应和改变老年人的健康服务使用行为模式为核心，引入共享决策，构建科学的偏好分析工具，以需求为导向，沿着服务可及、质量改善的思路，提出强化老年人社区健康服务有效供给的策略。基于循证基础，以整体健康观为视角，调查老年人的社区健康服务项目需求；基于偏好特征，立足个体异质性，结合随机参数Logit模型及潜类别模型，构建基于老年人偏好的离散选择实验分析框架，挖掘老年人对社区健康服务项目的深层次需求；在此基础上，纳入离散选择实验分析框架，考察老年人的理性与非理性的双重偏好，提出老年人社区健康服务的优化与助推策略，为政府提供相关决策依据。这不仅有利于初级医疗卫生资源的优化配置，更是大健康背景下人文关怀精神的重要体现。

慢性疾病的侵害和老龄社会的现实，对老年人社区健康服务提出了“数量”“种类”和“质量”的三重需求。然而，老年人社区健康服务供给总体不足与需求不断增长的矛盾十分突出。为此，本项目针对老年人社区健康服务中的供需错配问题，以适应和改变老年人的健康服务使用行为模式为核心，引入共享决策，构建科学的偏好分析工具，以需求为导向，沿着服务可及、质量改善的思路，提出强化老年人社区健康服务有效供给的策略。基于循证基础，以整体健康观为视角，调查老年人的社区健康服务项目需求；基于偏好特征，立足个体偏好的异质性，提出整合型的老年人社区健康服务项目需求框架，并借助最优最劣尺度法挖掘老年人对社区健康服务项目的深层次需求；在此基础上，考察老年人的理性与非理性的双重偏好，借助离散选择实验分析框架，优化老年人社区健康服务，并提出相关助推策略。研究结果发现，多重慢病（34.70%）和认知衰弱（56.89%）是老年人中最主要的健康问题，药物依从、健康促进生活方式、社会资本（社会网络、社会支持、社会参与等）与心理状况（孤独、抑郁）是老年集群档案中最重要的因素。疾病管理（疾病诊疗、康复护理）和药物管理（药物供应、合理用药）是老年人亟需的两大社区健康服务项目，而且从服务整合角度得出服务提供路径为：药物连续供应>疾病诊疗>康复护理>合理用药。“以患者为中心”、建立良好的医患信任，是服务质量的优化方向，在互联网快速发展的时代，通过互联网提升医疗服务的可及性、公平性与便捷性，是提升医患信任并助推老年人社区健康服务利用的重要策略。本项目从老年人需求的角度厘清了社区健康服务的供给方向，并从老年人偏好的角度表明了“以患者为中心”服务的重要性，且强调了老年人对互联网医疗的重视与渴求，为建立优质高效的老年人社区健康服务体系提供了决策参考。
