Being a refractory illness, the prevalence of systemic lupus erythematosus (SLE) is about 70 per 100 thousand, which seriously endangers people's health. Corticosteroids (GC) is the commonly used drug in treating SLE in Western medicine, but a lot of side-effects usually put doctors into a dilemma. In the long term study, we found that Lang-chuang-ding combined with GC has good effects in treating SLE. .In this study, we wish to clarify the regulating effects of Lang-chuang-ding on the distribution ,expression and nuclear translocation of GRα and GRβ in pDC and T cell of GC-sensitive or GC-insensitive SLE patients, and to detect the affinity of FITC-Dex and GRα by flow cytometry, and to detect the transcriptional activity by constructing restructuring plasmid with GC response element (GRE) genes and firefly luciferase (luc) reported gene plasmid, and to detect the test interactions of GR α and NF-к B p65 by immune total precipitation,therefore expound the regulation function of Lang-chuang-ding on affinity and transcription activity of GRα and GC and interaction of GRα and NF-к B p65,and to screening proteins interacted to GRβ, to clarify the regulation function of Lang-chuang-ding on GRβ and the interacted protein,so to analyze the synergism mechanism of Lang-chuang-ding for lupus,which may also to provide scientific data for traditional Chinese medicine treatment for SLE and provide drug targets.
系统性红斑狼疮(SLE)患病率约为70/10万,是严重危害人民健康的难治病,西医主要应用糖皮质激素(GC)等治疗,但存在副作用较多的困境。我们长期研究发现,狼疮定对GC治疗SLE具有较好的增效作用。.本项目拟通过阐明狼疮定对GC不同敏感性SLE pDC、T细胞GRα、GRβ分布、表达和核转位的调控作用;通过流式细胞术检测FITC-Dex与GRα的亲和力、构建GC应答元件(GRE)基因与萤火虫荧光素酶(luc)报告基因质粒检测转录活性、免疫共沉淀法检测GRα与NF-кB p65相互作用,阐明狼疮定对GRα与GC亲和力、转录活性及GRα与NF-кB p65互作的调控作用;通过筛选与GRβ互作蛋白,阐明狼疮定对GRβ及互作蛋白的调控作用,以明确狼疮定的增效机制,为中医药治疗SLE奠定科学依据,并提供药物作用靶点。
系统性红斑狼疮(SLE)是多器官受累的自身免疫病。糖皮质激素(GC)主要通过与糖皮质激素受体α(GRα)结合而调节相关基因表达以发挥生物学效应,GRβ可拮抗GRα而抑制GC发挥效应。狼疮定(LCD)结合GC治疗SLE具有较好的增效减毒作用。本项目从中医善于协调阴阳对立平衡的特色出发,初步阐明了LCD联合GC对CD4+T细胞GRα和GRβ调控作用。研究结果如下:(1)采用薄层色谱法和高效液相色谱法测定LCD的主要成分,有效的保证了本项目研究中的中药质量。(2)GC敏感型CD4+T细胞凋亡率明显高于GC抵抗型CD4+T细胞凋亡率。经DXM+LCD处理12h后,GC敏感型CD4+T细胞早期凋亡率和总凋亡率明显高于空白组和DXM组(均为P<0.01)。DXM+LCD含药血清处理12h后,GC抵抗型CD4+T细胞的早期凋亡率明显高于空白对照组和DXM组(均为P<0.01)。(3)GC不同敏感型SLE CD4+T细胞GRα蛋白表达存在差异:GC抵抗型SLE CD4+T细胞GRα的表达低于GC敏感型。(4)LCD可增加GC不同敏感型CD4+T细胞GRα表达:LCD+DXM处理24h后,GC敏感型CD4+T细胞的GRα表达水平高于DXM组(P﹤0.05);GC抵抗型CD4+T细胞内GRα表达水平高于DXM组和空白组(P﹤0.01,P﹤0.05)。(5)LCD降低CD4+T细胞GRβ表达:LCD+DXM处理后,GC敏感型和抵抗型CD4+T的GRβ表达均显著下降(均P<0.01),但对GC抵抗型的抑制作用低于对激素敏感型。(6)LCD增强GRα与GC亲和力、转录活性:在成功构建GRa过表达质粒转染细胞株后,通过双荧光素酶实验分析表明LCD可增强GRα的转录活性。(7)LCD能降低GRα与NF-кB p65的结合力:免疫共沉淀分析表明:DXM可使GRα与NFkappaB的结合力提高,影响DXM与GRα结合发挥疗效;而经LCD联合处理后,GRα与NFkappaB的结合力均明显减弱,且呈剂量依赖关系(均为P<0.01)。研究结果基本阐明狼疮定调控GRα、GRβ表达及功能而发挥增效作用,为SLE的中医药治疗提供科学依据。相关成果获省级科技进步奖一等奖1项,发表论文10篇,其中2篇Scientific Reports(IF:5.3),1篇Medicine(IF:2.13),培养硕士2名。
