基于利益均衡和制度整合的我国全民医疗保险体系构建和制度安排研究

It is a critical theoretical and practical issue in the reform of health insurance system to establish a national universal health insurance system with the orientation as fairness, justice and efficiency, and with the main content as the fairness of right, opportunity and rule, to achieve the integration of system and the balance of multi-stakeholder’s interests. This study follows the logic clue that “social policy atmosphere” → “problem posing” →“target building” → “solution posing” →“countermeasure analysis”, and on the basis of the systematic review and comparative analysis on the development process and key problems of universal health insurance system in China, and put forward the basic value judgment and ethical orientation of health insurance system with Chinese characteristic in line with the transition and development of economic and population in China. Then, it will utilize the combinational methods including theoretical model building, case analysis and empirical research, to establish the evaluation framework and index system of universal health insurance, so as to evaluate the effect of the universal health insurance system in China. Next, it will take the financing and payment system reform and medical insurance negotiation mechanism building as the breakthrough points, to formulate the pathway to realize the integration of funds, integration of urban and rural areas and integration of areas, integration of management institutions and information resources of the health insurance system, and finally construct the integration and optimization strategy for national universal health insurance system that adjusted to China national situation and with longitudinal and transverse function closely connected, so as to provide important theoretical reference and policy basis for the reform and project establishment of national universal health insurance system.

建立以公平、正义、高效为宗旨的，以权利公平、机会公平、规则公平为主要内容的全民医疗保险体系，实现制度整合，构建多方利益均衡格局，是我国医疗保险体系改革面临的重大理论与现实问题。本研究遵循社会政策氛围→问题提出→目标构建→提出方法→对策分析的逻辑研究思路，在对我国全民医疗保险制度发展历程和关键问题进行系统评述和比较分析的基础上，基于中国经济与人口发展的变迁，提出中国特色医疗保险系统的基本价值判断与伦理取向；采取理论模型构建、案例分析与实证研究相结合的方法，构建全民医保评价框架和指标体系，对我国全民医疗保险体系进行效果评价；以筹资与支付系统改革、医保谈判机制建立为切入点，制定我国医疗保险制度的基金统筹、城乡整合、地区整合、管理机构及信息资源整合的实现路径，最终构建符合我国国情、纵向功能和横向衔接的全民医疗保险制度整合与优化策略，为我国全民医疗保险体系改革及其方案制定提供重要理论参考和政策依据。

建立以公平、正义、高效为宗旨的我国全民医疗保险体系，实现制度整合，构建多方利益均衡格局，是我国医疗保险体系改革面临的重大理论与现实问题。.本项目成员2014-2018年期间发表并标注本项目来源与编号的成果：发表国际期刊论文（SCI/SSCI）10篇，中文核心期刊、CSSCI期刊论文54篇，出版中文著作9部。.在本项目资助下举办高端国际会议2场、赴境外参加国际会议8次；举办或参加国内高端学术会议39次；获中国医疗保险学术年会优秀论文5篇；.研究成果获湖北省社会科学优秀成果奖一等奖、中华医学科技奖卫生管理奖；.本项目在以下方面取得了较为重大学术和政策意义的创新成果：.1.基于中国国情将国际社会保障理念与中国健康政策与方针有机融合重构我国医疗保险价值理论体系；明晰了人民对健康保障的需要与医保发展不平衡不充分的主要矛盾；明确了中国特色全民医疗保险发展的新理念、新思路。.2.提出我国基本医疗服务与现行的基本医疗保险制度紧密相连，基本医疗保险为基本医疗服务的主要筹资渠道，据此制定了基本医疗服务诊疗项目包。.3.明确了人口政策变化与人口发展趋势、经济发展相关因素对医保基金平衡力和医疗保障水平存在显著影响，提出退休人员适宜缴费水平。.4.国内领先确定了基本医疗保险制度绩效评价核心指标体系及权重，对医疗保险基金的筹资、支付风险预判具有重要意义。.5.从新农合的筹资模式、国家政策导向等方面明确了适宜的新农合基金财务风险判定标准。.6.提出了优化异地就医制度设计、异地就医风险防范策略；.7.重视重大疾病早期的医保政策干预有利于降低医保基金支出、降低灾难性支出；将靶向药物纳入医保不会造成医保基金风险的增加，并且提高肿瘤患者的生命质量。.8.大病保险对于低医疗费用患者保护作用明显，但对高医疗费用患者保护作用有限；目录外费用是城镇居民家庭灾难性卫生支出发生的主要原因。.9.我国城镇职工基本医疗保险基金存在较大的潜在风险，应加快医保制度整合，增加医保基金风险防范能力，将个人账户与统筹账户打通使用，以提高基金使用效率。.10.提出了操作性强的我国医保谈判实务规则。.11.提出促进“医保、医疗、医药”有效衔接与全民医保制度构建策略。.基于本项目研究成果，获得了12项政策转化，其中包括中共中央政治局委员、国务院副总理孙春兰的重要批示；完成了9篇国家部委、省级部门及有关地方政府政策咨询报告。
