Our recent study found two signature miRNAs in schizophrenia circulating plasma --miR-130b and miR-193a-3p, which could serve as diagnostic biomarkers forschizophrenia. The aim of this study is to (1) assessing the resolving powers for miRNA biomarkers by chi-square test via comparing the miRNA levels between ultra-high-risk patients (UHR), schizophrenia patients, and healthy controls; (2) assessing the early warning abilities of these miRNA biomarkers via monitoring their dynamic levels in the follow-up study. The findings of this study will be helpful in early identification and early intervention for schizophrenia convertable UHRs, and will provide scientific basis and support to the precise diagnosis and treatment for schizophrenia.
本课题组在前期工作中,于精神分裂症患者外周血浆中发现miR-130b和miR-139a-3p能够作为精神分裂症的生物标志物,本申请拟:(1)通过精神分裂症超高危患者、精神分裂症患者和健康对照血浆miRNA相对含量的比较,通过卡方分析评估miRNA标志物对精神分裂症超高危患者的分辨能力;(2)通过对超高危患者的临床随访,监测血浆miR-130b和miR-193a-3p的动态变化情况,并结合临床表型数据和终点疾病转变率,评估miRNA标志物对疾病转化的早期预测能力。本项目的研究结果将有助于对具有精神分裂症转变潜质的超高危患者进行早期识别,早期干预,为精神分裂症的精确化诊疗系统的建
本项目完成对400名精神病超高危(Ultra High Risk,UHR)患者为期2年的临床随访,并检测了其基线期和终点期血浆miR-130b和miR-193a-3p的水平。研究发现,在UHR患者基线期血浆中,miRNA生物标志物水平较健康对照并未发生显著改变,远低于首次发作精神分裂症患者血浆中水平。在随访终点血浆中,miR-130b和miR-193a-3p的水平在临床转归组患者中显著上升,且与SIPS/SOPS量表总分及阴性症状评分显著正相关。但在临床未转归组患者血浆中,仅miR-130b水平出现一定升高,但在个体间一致性较低;miRNA标志物水平与SIPS/SOPS量表并无明显关联。研究结果表明,血浆miR-130b和miR-193a-3p具有作为UHR患者病程转归过程中的状态监控生物标志物,可以辅助临床医生对患者疾病状态的判断及早期干预方案的选择。
