The environmental pollution and high-caner-risk clusters in local regions of Huai River Basin has caused concerns of government and society. However, there exist significant diversities in types of cancer clusters among different regions. The breast cancer and thyroid cancer show high cancer cluster in some regions and the cause is unknown. Therefore, based on exposomes idea and technology, we focus on the innerexposomes of contaminants in human to identify the kinds and levels of contaminants in blood and urine samples of population through the detection method with GC-MS and LC-MS/MS. We fully use spatial epidemiology and GIS technology to reveal the characteristics of clusters of total cancer, breast cancer and thyroid incidence or mortality based on death surveillance data and cancer registry data collected continuously over the past decade. Using the evolutionary analysis of pollution based on exposomes to identify the similarities or divergence among inner-exopsomes of pollutants in population in different clusters. The effects of contaminants on cancer clusters would be investigated by cluster analysis and spatial analysis. The correlation between environmental pollution exposomes and breast cancer and thyroid cancer will be revealed and the source of pollution in biological samples will be traced for confirm the contaminants which is the potential cause of high-cancer-risk clusters and the effective control policy can be developed.
淮河流域环境污染与局部区域癌症高发聚集问题一直备受遍关注。然而,不同区域肿瘤聚集的类型存在较大差异,乳腺肿瘤和甲状腺肿瘤在某些区域呈高度聚集,但原因未明。本研究基于暴露组理念,以人群血尿生物样品中的污染物内暴露组特征分析为切入点,利用已建立的 GC-MS和LC-MS/MS导向的目标污染物检测方法,分析生物样品中污染种类和含量水平。充分利用以往收集的连续性死因监测数据和肿瘤登记数据等,采用空间流行病学和GIS技术分析人群肿瘤、乳腺和甲状腺肿瘤发病(死亡)聚集性特征。利用暴露组特征为基础的进化演化分析技术对不同聚集区人群污染内暴露组进行相似性比对和差异性分析,进而通过聚类分析和空间分析技术以阐释污染物在肿瘤聚集中的作用。建立淮河流域特征性环境污染物暴露与肿瘤特别是乳腺肿瘤和甲状腺高发聚集的关联,并对生物样品中污染物来源进行溯源分析,以明确引起肿瘤高发的污染物,制定有效控制策略。
研究按计划进行。按照课题预订计划建立了水中潜在甲状腺干扰物(替代阻燃剂)检测方法;开展了淮河肿瘤高发区S县潜在甲状腺干扰物(替代阻燃剂)地下水暴露组特征分析及其人群健康风险评估;基于现实暴露水平开展了替代阻燃剂细胞毒性和混合暴露的甲状腺干扰效应评价;建立了人群血、尿中潜在甲状腺干扰物代谢物(有机磷阻燃剂等)原型和代谢物的检测方法;并完成了人群甲状腺干扰物内暴露与甲状腺干扰效应关系研究。发表第一标注SCI论文4篇,发表第一标注中文论文2篇。
