Infiltration and metastasis are the foremost biological characteristics of pancreatic cancer in the early stage,about more than 90% patients already have local or distance mtastasis when at clinic diagnosis,although more than half of them have been effective treated,finally die of infiltration and metastasis. The protease secreted by tumor cells have a hand in the degradation of extracellular matrix(ECM),this is the most important step of tumor cells' infiltration and metastasis, hypostatic tumor ubiquity have areas lack of oxygen.The exist of these hypoxic cells is the material cause of hypostatic tumor tolerate chemotherapy and radiotherapy and also of tumor' s relapse and metastasis.Overcome the hypoxia of tumor is one of the important research subjects of tumortherapy.Tissue factor pathway inhibitor-2(TFPI-2) is a kind of serine protease inhibitor,it plays a important role in keeping the structure of extracellular matrix(ECM) integrality, this research use oxygen carrying ultrasound microbubbles to change the oxygenic environment of tumor cells , Through the establishment of the nude mice corneal microcapsule transplantation model,pancreatic adult cell tumor.transfer model to reseach the influence of TFPI-2 genes in oxygen-rich, hypoxic state to pancreatic cancer cell growth,angiogenesis ,,vivo and vitro invasion. Investigate the relationship between TFPI-2 genes of different oxygen environment with pancreatic cancer invasion and metastasis, provide the experimental basis for gene therapy of pancreatic cancer.
浸润和转移是胰腺癌早期最重要的生物学特性,约有90%以上的病人在临床确诊时已发生了局部或远处转移,超过半数的病人虽然经过有效治疗,但最终死于侵袭与转移,肿瘤细胞分泌的蛋白酶参与的细胞外基质(ECM)的降解,是肿瘤细胞实现浸润转移过程中的关键一步,实体肿瘤普遍存在氧缺乏区域,肿瘤乏氧细胞的存在是实体肿瘤对化疗、放疗耐受,以及肿瘤复发、转移的主要原因,克服肿瘤乏氧,是目前抗肿瘤治疗的重要研究课题之一,组织因子途径抑制物-2(TFPI-2)是一种丝氨酸蛋白酶抑制物,在维持ECM的结构完整性方面起着重要作用,本研究利用超声载氧微泡技术,改变细胞生长的氧环境,通过建立裸鼠角膜微囊移植模型、胰腺癌成瘤、转移模型,研究TFPI-2基因在富氧、乏氧状态下对胰腺癌细胞生长、血管生成、体内和体外浸润能力的影响,探讨TFPI-2基因不同氧环境与胰腺癌浸润转移之间的相互关系,为胰腺癌的基因治疗提供实验依据。
细胞外基质(Extracellular matrix, ECM)的降解是浸润和转移的一个重要步骤,研究表明一种锌离子依赖的蛋白酶,基质金属蛋白酶(Matrix metalloproteinase, MMP)-2,能促进ECM的降解,从而促进肿瘤浸润和转移。组织因子途径抑制剂(Tissue factor pathway inhibitor, TFPI)-2是一种丝氨酸蛋白酶抑制剂,它能够抑制各种蛋白酶的表达和活性,包括MMP-2,被认为是ECM免遭降解的保护者,从而可以对抗肿瘤的浸润和转移。先前的研究也表明MMP-2和TFPI-2在肿瘤组织中的表达呈相反趋势,且TFPI-2的抗肿瘤侵袭特性与MMP-2表达的减少密切相关。MMP-2和TFPI-2在肿瘤组织的异常表达不仅仅与肿瘤发生密切相关,也与肿瘤浸润、转移及血管生成密切相关。先前的研究已经证实MMP-2在胰腺癌组织中呈高水平表达,而TFPI-2在胰腺癌组织中呈低水平表达。然而二者在胰腺癌组织差异性表达的相关性以及临床意义目前并不清楚。鉴于此,衍生了本研究。本研究采用免疫组化技术检测胰腺癌组织与相应的癌旁正常胰腺组织中MMP-2和TFPI-2的表达情况。统计分析二者在胰腺癌组织中的表达水平及相关性,探讨其与肿瘤血管生成、临床病理参数、术后早期复发及生存时间的关系。我们的研究结果表明了MMP-2和TFPI-2在胰腺癌组织中的表达呈负相关性。MMP-2的高表达和TFPI-2的低表达与肿瘤血管生成、恶性的临床病理特征、术后早期复发及缩短的生存期密切相关。MMP-2和TFPI-2的联合检测可能是预测术后胰腺癌病人不良预后的一个潜在模型。
