基于多类型患者需求及关键资源约束的门诊预约递阶控制机制与动态调度方法研究

Considering the public having the urgent needs of healthcare service system, the project aims to study the outpatient appointment scheduling problem to realize the optimal allocation of healthcare resource. The project will build a multi-timescale-oriented outpatient appointment scheduling architecture and decompose hierarchically the healthcare resource to the different timescales considering the medical key resource constraint. According to the combination of the double-preference of outpatients and utilization balance of healthcare service system, the hybrid scheduling approach using fuzzy cognitive map and genetic algorithm will be presented to realize the dynamic appointment scheduling. The simulation-based optimization is used to realize the on-line scheduling considering the stochastic event distributions. This research and resulting conclusions will help to implement the effective appointment and efficient scheduling of healthcare resource. Furthermore, the expected result will improve the public satisfaction and operation efficiency and propel the further development of healthcare revolution.

本项目旨在迎合中国医疗改革的深入发展以及公众对医疗服务系统的迫切需求，以医疗资源的优化配置为主要内容，根据医疗服务系统的网格性、复杂性、随机不确定性、混合性等各种特性，考虑多类型的病患需求以及医疗关键资源的约束，针对医疗服务系统的预约控制机制和动态调度方法展开研究。拟建立面向多时间尺度的门诊预约调度体系，并在医疗关键资源动态需求规划的支持下，引入收益管理理论将医疗服务系统的服务能力递阶分解到不同的时间尺度上；基于患者对于时间和主治医生的双重偏好和负载均衡，对于门诊预约进行动态调度；最终在仿真优化平台的支持下，实现医疗服务系统对于急诊等随机事件的实时处理。本项目的研究将丰富服务系统的先进计划与调度管理理论，促进工业工程在服务行业的应用，研究成果对于提高我国医疗服务机构的公众满意度和运作管理效率，促进医疗改革的深入发展将起到直接的推动作用。
