Auto-immune dry eye disease (DED) would damage the ocular surface health and influence the quality of life.Its mechanism may depend on excessive activation of PI3K/Akt/mTOR pathway and unbalance of Treg/Th17 cells.Qinpi eyedrop is a traditional Chinese herb drug.In our previous study,we found its primary element Esculetin could improve the tear indexes,downregulate the IL-1α,IL-1β,TNF-αexpressions of ocular surface tissues.However,the exact anti-inflammatory mechanism was unclear.In this study,we cultured Treg/Th17 cells under hyperosmolar enviroment in vitro and constructed auto-immune DED rabbit models.Treg/Th17 cells proliferation and apoptosis,inflammation index (IFI),morphology alternations,Treg/Th17 ratios, specific transcription factors Foxp3,ROR-γt and Akt expressions were observed to probe Qinpi eyedrop regulating the balance of Treg/Th17 cells through inhibiting PI3K/Akt/mTOR pathway and provide a new approach of traditional Chinese therapy for auto-immune DED.
自身免疫性干眼是危害眼表健康、影响生活质量的一类眼表疾病,其发病机制可能与PI3K/Akt/mTOR信号通路的过度激活及Treg/Th17细胞的失衡有关。秦皮滴眼液是传统中药制剂,具有抗炎的功效。课题组前期研究发现:秦皮滴眼液所含的主要有效成分——秦皮乙素可有效改善干眼模型兔的泪液学指标、下调眼表组织中多种炎症因子IL-1α、IL-1β、TNF-α等的表达,但具体作用机制仍不明确。本研究中,我们通过在体外高渗环境下培养Treg/Th17细胞和建立兔自身免疫性干眼模型,观察秦皮滴眼液对细胞增殖与凋亡、眼表炎症指数、泪液学指标、组织形态学、Treg/Th17细胞比率、转录因子Foxp3、ROR-γt及通路Akt的mRNA和蛋白表达,探究秦皮滴眼液通过抑制PI3K/Akt/mTOR信号通路调控Treg/Th17细胞平衡治疗自身免疫性干眼的机制,为中医药治疗干眼提供新的途径。
自身免疫性干眼是危害人类眼表健康、影响生活质量的眼表疾病,具有病情反复、迁延不愈的特点,目前缺乏疗效肯定、质优价廉的眼部治疗药物。秦皮滴眼液能缓解干眼患者眼部干涩、眼红等不适,但具体机制仍不明确。自身免疫性干眼可能与眼表PI3K/Akt/mTOR信号通路过度激活、Treg/Th17细胞失衡及相应特异性转录因子Foxp3、ROR-γt异常表达有关。本项目通过体外高渗培养人外周血单核淋巴细胞(PBMC),予秦皮乙素(Esc)干预并观测细胞存活率、凋亡率、Treg、Th17细胞比例、PI3K、AKT、Foxp3、ROR-γt表达;建立兔自身免疫干眼模型,予秦皮滴眼液(QP)干预并观测泪液学指标、眼表炎症程度、组织形态学、泪腺ROR-γt、Foxp3免疫组化、AKT、Foxp3、ROR-γt表达。体外研究显示:与模型组比较,Esc组PBMC存活率、Treg比例升高、Foxp3的mRNA和蛋白表达均升高,细胞凋亡率、Th17比例、PI3K、AKT、ROR-γt的mRNA和蛋白表达降低(均P<0.05);体内研究显示:与模型组比较,QP组兔泪膜破裂时间延长、泪液分泌量升高、角膜荧光素染色评分、眼表炎症指数评分降低,角膜、结膜、泪腺炎症浸润程度减轻,泪腺Foxp3免疫组化、mRNA和蛋白表达均升高,泪腺ROR-γt免疫组化表达、PI3K、AKT、ROR-γt的mRNA和蛋白表达均降低(均P<0.05)。综上结果说明秦皮滴眼液通过抑制PI3K/Akt/mTOR信号转导通路调控Treg/Th17平衡及Foxp3、ROR-γt异常表达,从而达到治疗缓解自身免疫性干眼的目的,为临床自身免疫性干眼治疗药物的探索提供参考。
