Xerostomia after radiotherapy in head and neck cancer seriously affects the patients’ life quality. Recently in our NIH-supported international multi-center randomized controlled clinical study with the United States, we reported for the first time that acupuncture for the treatment of head and neck cancer after radiotherapy xerostomia in Cancer magazine does have effect. In order to confirm the effect of acupuncture, the molecular mechanisms need further investigation. We hypothesized that: acupuncture effective early release AchR effect by the sympathetic nerve, and later growth factor for capillary blood for therapeutic purposes reduce tissue fibrosis. The project intends to employ cell biology and molecular biology techniques, adopt mature rats radiotherapy model, taking CV24、LU7、KI6 as the acupuncture points, and observe the number and doses of different radiation therapy of salivary glands function and acupuncture efficacy and the dynamic changes about day1、day3、day5、day7、day14 after the acupuncture. Saliva viscosity, PH, serous cells, ion transport, Ach, and transformed positive growth factor-β1 are indicators. We will investigate the molecular mechanisms of acupuncture treatment of xerostomia, in order to provide a theoretical basis for clinical treatment, allow the effect of acupuncture to gain scientific understanding and international recognition.
目前头颈部肿瘤放疗后口干问题严重影响了患者生存质量,我们前期在NIH支持下与美国开展 国际多中心随机对照临床研究,在Cancer杂志首次报道针刺对治疗头颈部放疗后口干确有疗效。为了明确针刺疗效,其分子机制有待深入研究。我们提出假说:针刺产生的有效信息早期通过兴奋副交感神经,释放AchR产生作用,后期通过生长因子促进微血管血供,减少组织纤维化来达到治疗目的。本项目拟采用细胞生物学与分子生物学技术,选用成熟的大鼠放疗模型,取"承浆"、"列缺"、"照海"为针刺穴位,观察不同放疗次数和剂量下对唾液腺功能的影响和针刺疗效,以及针刺治疗结束后第1天、3天、5天、7天、14天的动态变化。以唾液粘度、PH值、浆液性细胞、离子转运、Ach以及转化正长因子-β1为指标。挖掘针刺治疗口干的分子机制,为临床治疗提供理论依据,使针刺疗效得以科学的理解,取到国际认同。将结果指导临床,使针灸在肿瘤领域发挥作用。
