Type 2 diabetes mellitus belongs to wasting-thirst of traditional Chinese medicine. Insulin resistance (IR) generally exist in it. Its principal pathogenesis of chinese medical science is body fluid deficit and dryness-heat abundance. Which is displayed metabolic disorder of glucolipid by disorder of glucose uptake, which is related with the disturbance of intracellular PI3K/Akt signal pathway of glucose transport. Accommodation of disturbance of glucose uptake by adiponectin and glucose transporter 4 causes metabolic disorder of glucolipid, which would result in insulin resistance (IR) of type 2 diabetes mellitus. We discovered that gegen qinlian decoction signi?cantly decreased fasting blood glucose of diabetic rats, and serum of gegen qinlian decoction increased glucose consumption, TG content and the concentration of secreted adiponectin protein. It is important scientific problem that how gegen qinlian decoction affect intracellular PI3K/Akt signal pathway of glucose transport by multitarget. So we propose hypothesis that the effect of anti- insulin resistance of type 2 diabetes mellitus is related with the signal pathway adiponectin/AMPK/PI3K/Akt/GLUT4.We will observe the mechanism of signal transduction of insulin resistance of type 2 diabetes mellitus, and the effect of Gegen Qinlian Decoction by animal model and two representative cell model (3T3-L1 adipocytes and L6 skeletal muscle cells),and by using a serum pharmacological method, Confocal laser scanning microscope technology and so on. Which would provide evidence of theory and experiment for development and clinical application of gegen qinlian decoction, and provide new target for prevention and cure of IR.
2型糖尿病属中医"消渴证"范畴,其普遍存在胰岛素抵抗(IR),多由长期过食肥甘,致燥热伤津,主要表现为细胞葡萄糖摄取障碍导致糖脂代谢紊乱,与调节葡萄糖转运的PI3K/Akt通路有关。我们发现,葛根芩连汤能降低血糖,其含药血清能增加脂肪细胞葡萄糖消耗量、脂联素量及膜GLUT4蛋白量,那么葛根芩连汤如何多靶点多环节影响葡萄糖转运的PI3K/Akt通路的,这是一个重要科学问题,本课题提出"葛根芩连汤治疗IR作用,可能与其多靶点多环节作用于adiponectin/AMPK/PI3K/Akt/GLUT4信号通路,调节糖脂代谢有关"(假说);拟通过动物及两种代表性细胞模型(3T3-L1脂肪细胞及L6大鼠骨骼肌细胞),结合血清药理学、激光共聚焦等方法和技术,观测清热生津经典名方"葛根芩连汤"治疗IR作用及其PI3K/Akt信号机制。可为葛根芩连汤开发和临床应用提供理论及实验依据,为防治IR提供新靶点。
2型糖尿病属中医“消渴证”范畴,其普遍存在胰岛素抵抗(IR),多由长期过食肥甘,致燥热伤津,主要表现为糖脂代谢紊乱,与PI3K/Akt通路有关。葛根芩连汤是经典名方,其治疗2型糖尿病胰岛素抵抗作用临床疗效确切。我们在现有基础上,分析了葛根芩连汤治疗IR作用可能信号转导机制,取得如下成果:(1)葛根芩连汤可显著降低2型糖尿病胰岛素抵抗大鼠空腹血糖、糖化血清蛋白浓度及糖化血红蛋白水平;(2)可降低 KK-Ay糖尿病小鼠的体重、空腹血糖、甘油三酯、总胆固醇,以及降低胰岛素含量和升高高密度脂蛋白含量;(3)可降低 KK-Ay小鼠血浆LPS、TNF-α及IL-6含量,且可调节KK-Ay小鼠肠道菌群结构;(4)可增加3T3-L1脂肪细胞模型葡萄糖消耗量、甘油三酯含量、脂联素分泌量;(5)葛根芩连汤及其主要成份可调节L6骨骼肌细胞IR模型糖代谢。(6)葛根芩连汤含药血清可增加p-PI3K、Akt、p-Akt、p-AMPK、膜GLUT4蛋白的表达(P<0.05);(7)葛根芩连汤含药血清作用HepG2细胞IR模型,能显著增加葡萄糖消耗量和糖原含量,降低PEPCK活性。(8)从蛋白质组学水平分析,上调蛋白PI3K(Q63788)、IRS(P35570)等以及下调蛋白Apo C-III (P06759)、FABP1(P02692)等差异蛋白可能参与了胰岛素信号转导等通路。(9)此外,我们发现葛根芩连汤的主要成份小檗碱有一定的降糖作用,葛根素对 HepG2 细胞IR有改善作用。通过本项目研究,有助于葛根芩连汤进一步开发应用,有助于古方新用,可为葛根芩连汤开发和临床应用提供理论及实验依据,为防治IR提供新靶点,因此,具有十分重要的意义。另外,我们对葛根芩连汤的主要活性组份联合用药进行了相关研究,为葛根芩连汤的配伍或多靶点作用机制研究提供了实验数据。
