真实世界大数据驱动的全景式健康医疗管理与服务模式研究

The traditional health management model centered on “disease treatment” cannot meet people's needs. The real world health medical big data accumulated by various monitoring, medical and scientific research needs to be developed and utilized. The project aims to establish a real-world health medical big data integration platform and a panoramic health care new management and service model demonstration platform, including “Health Medical Big Data Pool” which was based on national monitoring systems, hospital electronic medical records, community resident’s health records, crowd queues, network and environmental data; multi-source data sharing mechanism, and a panoramic management mode demonstration platform for artificial intelligence enablement, data sharing driver and business process specification. .And on this platform, we will first demonstrate a panoramic prevention and control mechanism for infectious diseases based on multi-source heterogeneous social data. Second, a new model of chronic disease diagnosis and management based on omics, clinical and population cohort will also be demonstrated. And last other health care-related projects supported by the Big Data Driven Management and Decision Research Major Research Program can be integrated. Finally, the evaluation of the effects, benefits and industrial value of the new panoramic management platform and model will provide a scientific basis for the establishment of a new human health management model. The proposed integration program will promote the development of disease prevention and health management systems based on real world big data and enrich the data-driven management paradigm theory.

以疾病治疗为核心的传统健康管理模式不能满足人们的需求，各种监测、医疗和科研积累的真实世界健康医疗大数据亟待开发利用。本项目拟建立一个真实世界健康医疗大数据集成平台和全景式健康医疗新型管理和服务模式示范平台，包括基于国家监测系统、医院电子病历、居民健康档案、人群队列、网络与环境数据的健康医疗大数据池，多源数据共享机制，以及人工智能使能、数据共享驱动和业务流程规范的全景式管理模式示范平台。并在该平台上，示范以多源异构全社会数据为基础的传染病全景式防控和健康干预机制，以及基于组学、临床和人群队列的全过程慢病诊疗与管理新模式，并可集成“大数据驱动的管理与决策研究重大研究计划”支持的其它健康医疗相关项目。最终评估全景式新管理平台和模式的效果、效益和价值，为构建以人为核心的新型健康医疗管理模式提供科学依据。所提集成方案将促进基于真实世界大数据的疾病防控和健康管理体系发展，丰富数据驱动的管理范式理论。
