Phacoemulsification and intraocular lens implantation is the main surgical approach to treat cataract. On the basis of inflammatory cytokine antibody array, our preliminary study revealed that the expression of a pain-related inflammatory chemokine, monocyte chemotactic protein 1 (MCP-1) was significantly elevated in the aqueous humor in the contralateral eye after the first-eye cataract surgery. Moreover, cataract patients were likely to have more pain during second-eye surgery than during first-eye surgery. These findings proposed the hypothesis that “MCP-1 overexpression is associated with intraocular inflammatory response and pain after cataract surgery”. This project intends to take the rabbit model as basis to investigate the influence of MCP-1 overexpression on intraocular inflammatory response of the contralateral eye, and interfere with the inflammatory response using MCP-1 antagonist in the in vivo experiments. Moreover, CCL2 knockdown mouse will be applied to study the effect of MCP-1 suppression on the inflammatory response in the contralateral eye after cataract surgery. The purpose of this project is to clarify the role of MCP-1 overexpression after cataract surgery in the early-stage intraocular inflammatory response, and construct a basis for its prevention and intervention.
超声乳化吸除联合人工晶状体植入术是目前白内障的主流手术方式。本项目组前期炎症因子抗体芯片研究发现,一眼白内障术后,对侧眼术前房水中单核细胞趋化蛋白(MCP-1)含量升高,同时患者第二眼白内障手术的主观疼痛评分也显著提高。因此提出“房水中MCP-1高表达失衡与白内障术后对侧眼炎症反应及疼痛相关”的假说。本项目拟扩大临床样本量验证前期发现;进一步采用家兔白内障手术模型,观察MCP-1高表达对白内障术后对侧眼早期炎症反应的影响,同时使用MCP-1表达受到抑制的CCL2基因敲除小鼠,进一步研究抑制MCP-1表达对白内障术后对侧眼早期炎症反应的影响;最后使用MCP-1拮抗制剂进行活体干预研究,探究该制剂对白内障术后对侧眼早期眼内炎症反应的干预效果。从而探究MCP-1高表达在白内障术后早期对侧眼炎症反应中的机制,并为预防和干预奠定基础。
超声乳化吸除联合人工晶状体植入术是目前白内障的主流手术方式,研究白内障患者术后眼内炎症反应程度,分析其相关影响因素,能够为白内障患者术后的眼内炎症评估及预防控制提供更有效的帮助。前期研究发现与第一眼白内障手术相比,对侧眼(即第二眼)白内障手术时患者感觉疼痛的比例显著增加,且第二眼围手术期疼痛程度与术前房水中MCP-1含量显著相关,因此提出"房水中MCP-1高表达失衡可能与白内障术后对侧眼炎症反应及疼痛相关"这一假说。本项目扩大临床样本量成功验证了前期发现,阐明了白内障术后房水中MCP-1的变化规律,即一眼白内障术后,对侧眼术前房水中MCP-1含量升高,同时患者第二眼白内障手术的主观疼痛评分也显著提高。进一步对动物模型进行双向干预后观察,发现MCP-1高表达对家兔白内障术后对侧眼早期炎症反应有促进作用,同时使用MCP-1表达受抑制的CCL2基因敲除小鼠,发现抑制MCP-1表达对白内障术后对侧眼早期炎症反应有抑制作用,并揭示了白内障术后房水中MCP-1高表达对眼内炎症因子表达的影响;最后使用MCP-1拮抗制剂进行活体干预研究,体现了该制剂对白内障术后对侧眼早期眼内炎症反应的干预效果及对白内障术后眼内炎症因子的影响。因此通过本课题探索,我们成功揭示了MCP-1高表达在白内障术后早期对侧眼炎症反应中的机制:患者行第一眼白内障术后存在的交感性刺激,可导致对侧眼白内障术前房水中MCP-1高表达其炎症诱导作用增强,导致了眼内微环境的改变,这一失衡表现呈“亚临床”眼内炎症反应状态。而在短期内行第二眼手术时,患者前房水中MCP-1仍处于高表达状态,因而二次手术刺激导致的围手术期炎症反应更为迅速,故患者常常主诉第二眼手术时出现不同程度的疼痛。因此,平衡MCP-1在房水中的表达量,有助于避免白内障术后早期眼内炎症的发生或抑制其发展。本课题的科学意义在于为进一步干预白内障术后早期眼内炎症反应奠定基础和提供实验数据支持,MCP-1局部滴眼制剂可能是药物干预白内障围手术期眼内早期炎症反应的新策略。
