提升基层医疗卫生服务能力研究

Healthy China 2030 Planning seeks to improve the service capacity of primary-level healthcare institutions as one of its core strategy. Currently, the existing literature lacks an explanatory theory composed of the concept and category of primary-level healthcare institutions’ service capacity, and most of the capacity assessments mainly focuses on static and structural resource capability of service provision. The empirical research on its impact factors is based on small-sample, and the suggestions on how to improve the service capacity are unsystematic. This study aims to a）develop a theoretical model to explain the service capacity of primary-level healthcare institutions from integration perspectives of process and structure, dynamic and static, demand and supply, and b) give systematic and elastic suggestions on how to improve it based on large-sample data collection and nationwide survey. The study consists of four parts logically: 1) to build a theoretical model to explain the concept and category of the service capacity of primary-level healthcare institutions, and accordingly develop an evaluation index system to measure it; 2) to propose and then test our given hypothesis about the impact factors on various categories of primary-level healthcare institutions’ service capacity; 3) to develop an evaluation index system of primary-level healthcare institutions’ service performance, and then run a multiple regression analysis of the data on capacity and performance; 4) to offer systematic and elastic suggestions to improve the service capacity of primary-level healthcare institutions through multiple case studies of various health reform practices in local governments. This study will be of great importance for the promotion of theoretical and empirical research and deepening primary-level healthcare service reform in China.

提升基层医疗卫生服务能力是“健康中国建设”的核心战略。目前，已有研究缺乏关于基层医疗卫生服务能力的概念及其分类构成的解释性理论，能力评估较多停留于静态的结构性资源能力，影响因素的研究不够全面且缺乏大样本的实证支撑，对策建议也相对碎片化。本项目基于全国性的大样本数据采集和调研，从过程与结构、动态与静态、需求与供给的整合视角，建立一个解释基层医疗卫生服务能力及其提升的理论模型并提出系统的对策建议。具体包括：（1）构建基层医疗卫生服务能力及其分类构成的理论模型，建立能力评价指标体系并测量；（2）针对基层医疗卫生服务能力各个分类的主要影响因素提出命题假设并进行科学检验；（3）构建基层医疗卫生服务绩效评价指标体系，基于大数据采集，对基层医疗卫生服务能力的各个分类与绩效结果的相关关系进行多元回归分析；（4）结合地方医改实践的多案例比较研究，提出提升基层医疗卫生服务能力的具有系统性和选择弹性的对策建议。

提升基层医疗卫生服务能力是“健康中国”战略的内在要求，更是深化医疗卫生体制改革的核心任务，亦是“以人民健康为中心”共建共享优质医疗卫生服务的重要路径。本项目以“强基层”为目标，紧扣基层医疗卫生服务能力提升的基础科学问题，以具有中国特色的整合型医疗卫生服务体系建设与县域医共体改革为关键环节，充分利用多学科交叉综合研究的优势，研究重点是：（1）基层医疗卫生服务能力的界定、分类与理论模型；（2）基层医疗卫生服务能力的测量与指标体系构建；（3）基层医疗卫生服务能力的影响因素与实证分析；（4）基层医疗卫生服务能力的绩效评价、实现路径与对策建议。经过五年的持续深入研究，积累了一批高质量研究数据库，形成了一系列具有原创性的研究成果，并实现了较高的成果转化及应用，同时形成了一支具有正确理念、掌握先进方法、年龄和知识结构合理的研究团队，并搭建了一个理论与实务深入沟通交流的平台。五年来，基于本项目核心思想、基本观点和重要结论，课题组提出了关于基层医疗卫生服务能力提升的一系列建议，引起中央领导或有关部门、有关省份领导的重视和肯定，部分成果供国家立法和重要政策制定参考、被用于中央部委或省级政府决策参考。截至2022年12月，本项目共发表学术论文22篇，其中中文期刊论文共16篇，英文期刊论文共6篇。所涉及的期刊包括《中国社会科学》《公共管理学报》《中国行政管理》《治理研究》Health Economics, Policy and Law、Journal of Chinese Governance等。此外，依托本项目完成博士学位论文2篇，硕士学位论文6篇。依托本项目出版书籍5本。总之，本项目全面回答了课题设计提出的科学问题，回答了基层医疗卫生服务能力提升的若干重大现实问题，若干重要成果已经被用于制度政策制定，已经实现了项目预设的研究目标。
