Scalp vitiligo is resistance to treatments. The pathogenesis is not clear now. Patients of vitiligo involving scalp were treated by hair transplantation.Our early with hair transplantation in the treatment of scalp vitiligo, skin color was found satisfactory, still keep hair white; for hair follicle melanocyte differentiation related genes by immunohistochemical staining found neural crest stem cells differentiate into melanocytes blocked pathway. PAX3 is a key factor in the regulation of the formation of a number of signaling pathways, and combined with the results of our previous studies, it is speculated that it plays an important regulatory role in the pathogenesis of vitiligo..This research focuses on the role and mechanism of PAX3 and related signaling pathway in the differentiation of HF-NCSC to mature hair follicle melanocytes ;evaluation on UVA1 in the treatment of scalp vitiligo ,which have important value on the theoretical system of the vitiligo pathogenesis and clinical application prospect by therapeutic target finding.
头皮白癜风目前缺乏明确有效的治疗手段,且发病机制还不清楚。我们前期通过毛发移植治疗头皮白癜风,发现表皮复色满意,毛发仍然保持白色;行毛囊部位黑素细胞系分化调控相关基因免疫组化染色,发现毛囊神经嵴干细胞定向分化为黑素细胞的通路受阻。PAX3是具有双向调控黑素细胞生成功能的多条信号通路中的关键因子,结合我们前期研究结果,推测其在白癜风发病过程中起重要调控作用。本研究旨在探讨和明确PAX3及其相关信号通路在人类毛囊神经嵴干细胞定向分化为成熟毛囊黑素细胞的作用和机制,同时评价UVA1在头皮白癜风的治疗效果。这些研究对于拓展和完善白癜风的发病机制理论体系,寻找合适的治疗靶点都具有重要的理论价值和深远的临床应用前景。
背景:发生在头皮部位的白癜风表现为头皮局部区域的毛发全部变白,同时伴有头皮脱色,常用的治疗方法往往极难复色。目前已发现毛发含有的色素依赖有活性的毛囊黑素细胞产生的黑素颗粒,而有活性的毛囊黑素细胞主要来源于毛囊隆突(bulge)部位储藏的神经嵴干细胞(Hair Follicle-Neural Crest Stem Cell, HF-NCSC)。既往有关毛发部位色素细胞谱系相关研究大多限于研究方法和取材所限,绝大部分文献均是以小鼠作为研究对象得出的。本研究拟通过对人类毛囊bulge部位的HF-NCSC细胞向黑素细胞谱系定向分化的研究,以期阐明白癜风白发的发病机制。.主要研究内容:取白癜风白发标本,分为毛囊漏斗部,隆突和毛球三个部分,对目标基因以及不同黑素生成通路在不同标本、不同部位组织中进行检测;取白癜风白发新鲜组织,仍分为毛囊漏斗部、隆突和毛球三部分,和正常毛囊培养的HF-NCSC共培养,以正常毛发为对照;将培养出的HF-NCSC给予窄波紫外线B(NB-UVB)干预,观察其分化迁移功能的影响;将白发使用环钻提出再将患者本人的正常毛发植入白癜风区域,观察到皮肤得到满意复色,且植入毛发保持黑色。.重要结果:毛发白癜风中HF-NCSC向黑素细胞的定向分化下调;NB-UVB对HF-NCSC向黑素细胞的分化和迁移能力起到促进作用,HF-NCSC可分化为成熟黑素细胞并迁移。.科学意义:通过对人类毛囊bulge部位的HF-NCSC细胞向黑素细胞谱系定向分化的研究,以期进一步阐明白癜风白发的发病机制,同时探索新的治疗靶点,开发有效治疗黑素细胞生成受损的治疗药物。
