Branch canals usually stem from the main root canal during its course. When pulp necrosis or infection happens, similar histo-pathological changes could be found in the soft tissues within branch and sub-branch canals, which could lead to periapical and periradicular inflammations. Researches show that the bacteria and necrotic tissue residues are basically similar before and after the canal preparation. Due to the uncertainty about the anatomical variation properties of brach canals, researches on the characterization of bacteria and infection control strategies in branch canals are seriously hampered. In this proposed study, Micro-CT will be used to re-construct branch canal morphologies in different teeth on which a comprehensive database for branch canals will be established and anatomical properties of branch canals will be clarified. Furthermore, biofilm models of Enterococcus faecalis in branch canals with different anatomical properties will also be established to facilitate the studies on the most effective concentration and time of silver nanoparticles for the infection control in branch canals. It is expected in this proposed study that new infection control methods effective for branch canals could be developed, which would have great significances for the prevention and treatment of branch canal-infection-related periapical and periradicular pathoses.
主根管在走形过程中会出现分支,形成分支根管。当牙髓出现感染或坏死时,分支根管中的软组织会出现与牙髓类似的表现,并可能导致根尖周和根侧组织的炎症。研究也证实,根管预备后分支根管内残留的坏死组织和细菌与预备前基本相同。由于分支根管形态变异的特点尚不清楚,分支根管内微生物感染的特征及感染控制方法的研究进展缓慢。本项目拟通过显微CT技术,重建出不同牙位多个牙的分支根管形态,构建出分支根管数据库,明确分支根管形态的变异规律。在此基础上构建不同种类分支根管粪肠球菌感染的生物膜模型,研究纳米银对分支根管粪肠球菌感染的控制效果。该项目的研究有望探索出不同种类分支根管感染控制的新方法,对于预防和治疗分支根管感染相关的根尖周或根侧病变具有重要意义。
主根管在走形过程中会出现分支,形成分支根管。当牙髓出现感染或坏死时,分支根管中的软组织会出现与牙髓类似的表现,并可能导致根尖周和根侧组织的炎症。本项目通过显微CT扫描及三维重建技术研究了分支根管,构建了分支根管数据库,明确了分支根管的形态变异规律。在感染控制方面,本项目构建了分支根管E.faecalis生物膜感染模型,完成其微生态特征分析;筛选了纳米银对牙本质E.faecalis生物膜感染的有效控制方案;制备并表征了含银介孔生物玻璃、含银纳米介孔颗粒以及载洗必泰的介孔钙硅纳米颗粒,分析了其细胞毒性、抗菌能力以及体外矿化能力;评价了含银纳米介孔颗粒对牙本质的亲和性及其对分支根管粪肠球菌生物膜的感染控制效果。本研究为制定分支根管感染的控制策略提供了理论依据,对临床牙髓病的治疗具有重要指导意义。
