In patients with acute myocardial infarction (AMI), the guideline-recommended treatment is mechanical revascularization by percutaneous coronary intervention (PCI), which is effective at reducing mortality. However, a substantial proportion of patients with AMI develop chronic cardiac failure because of ischemia/reperfusion (I/R) injury. Recently, irreversible microvascular injury and subsequent intramyocardial haemorrhage are now thought to be crucial to the development of new therapeutic strategies to treat ischemia/reperfusion (I/R) injury. In the present study, we conducted an experimental I/R injury model with intramyocardial haemorrhage to evaluate the cardioprotection for combination of salvianolic acid B (SalB), the main active ingredient of Salviae miltiorrhizae, with ginsenoside Rg1 (Rg1), the main active ingredient of Panax notoginseng. And we further investigate the underlying mechanism relating with intramyocardial haemorrhage. Furthermore, a series of assays were employed to explore the potential mechanism of cardioprotection through biochemical, functional and histopathological analysis. SalB/Rg1is a potential medication with reasonable and effective indications for further clinical development.
目前,临床指南对急性心肌梗死推荐的首选治疗方案是冠状动脉介入治疗(PCI),虽然PCI在降低患者死亡率方面发挥巨大作用,依然有大量患者出现无复灌现象,最近的研究显示心肌内出血有望成为改善PCI复灌不良的治疗切入点。本项目前期在重大专项的资助下,确定了丹参中活性化合物丹酚酸B(SalB)和三七中活性化合物人参皂苷Rg1(Rg1)的最佳配比,并确定了SalB/Rg1组合的在心梗复灌动物模型上对心脏的保护作用,并发现SalB/Rg1组合具有显著降低心肌内出血的疗效。本项目基于前期雄厚的研究基础从心脏血管和心肌细胞两个方面开展组合药物抗心肌内出血的机理研究,分别从MMP-9和TNF-α凋亡坏死相关通路开展分子水平的机理研究,并在此基础上探讨SalB/Rg1组合药物的长期疗效。该研究将深化SalB/Rg1组合药物的作用机理研究水平,为丹参三七这一经典药对的现代开发做出努力和贡献。
本项目在明确SalB/Rg1组合的在心梗复灌动物模型上对心脏的保护作用,以及SalB/Rg1组合具有显著降低心肌内出血的疗效后,分别从冠脉血管和心肌细胞两个方面开展组合药物抗心肌内出血的机理研究,并在此基础上探讨SalB/Rg1组合药物的长期疗效。在心肌细胞保护方面,SalB/Rg1组合防治心肌细胞凋亡、抑制MMP-9活性、保护线粒体结构完整性、抑制肌小节断裂;在冠脉微血管保护方面,SalB/Rg1组合抑制毛细血管阻塞、降低炎症细胞浸润、抑制心肌内出血、降低微血栓形成。在本项目的资助下培养了3名硕士研究生,项目负责人作为通讯作者发表丹参/三七相关SCI研究论文3篇(Cardioprotection of salvianolic acid B and ginsenoside Rg1 combination on subacute myocardial infarction and the underlying mechanism.
