In recent years,trauma, idiopathic scoliosis ,retrogression, tumor of adolescent's have taken on a rising trend gradually, With adolescent's (aged 12-18)thoracolumbar vertebra morphological development, There were different among each age on mechanical change, location of pathogeny, clinical presentation, as well as diagnoses and treatment methods. However, there were few systematic fundamental clinical anatomy research reports to the whole thoracolumbar vertebra structure. Digital three dimensional (3D) reconstruction technology could provide clear, direct-observed, three-dimensional imaging information for basic research and clinical diagnoses and treatment. What's more, this technology had been applied in the measurement of anatomy, design of surgery, making navigation template, evaluation of post-surgery, biomechanical analysis, design of artificial tissues and organs, design of internal and external fixator, human engineering research and Tele-medicine and so on. So far, there were few studies on adolescent thoracolumbar vertebra. This research project would perform systematic digital three-dimensional morphological measurement, CT and MR 3D reconstruction quantitative among different patients of different age groups and gender groups, biomechanical measurement by multi-discipline application and research, to explore the characteristics of adolescent thoracolumbar vertebra morphological development, biological mechanical change, internal fixation technology and regularity between age and bone density ,to provide theoretical evidence for adolescent spine disease prevention, thoracolumbar trauma surgery,tuberculosis surgery and idiopathic scolisis diorthosis.
青少年胸腰椎创伤、特发性脊柱侧凸、退行性变、肿瘤等疾患日趋增多,12~18岁青少年处于青春发育高峰期,各年龄段正常及异常脊柱形态结构、生物力学特性、疾患的临床诊治均有较大差异,国内外对其临床应用解剖学系统研究较少。与传统研究方法相比,三维数字化技术不但可以为基础和临床研究提供直观、逼真的立体图像,更可在人体结构精准测量、各种临床术式和人工假体的设计规划、治疗效果评估、固定系统生物力学分析及远程医疗等方面具有很好的应用价值,但尚缺乏对该年龄段系统研究。课题拟行该年龄段青少年正常和特发性脊柱侧凸胸腰椎、椎间盘及周围血管神经分年龄、性别和侧别三维数字化临床应用解剖学测量,比较其差异,探索其正常和异常胸腰椎形态发育特征、内固定手术参数、设计及生物力学特征,为青少年胸腰椎疾患的预防,胸腰椎创伤、结核及退行性变手术,特发性脊柱侧凸矫正术等提供理论依据。
通过研究,基本总结了12~18岁青少年正常及特发性脊柱侧凸(AIS)胸腰椎、椎间盘及周围血管不同年龄、性别和侧别三维数字化临床应用解剖学及生物力学变化规律;提供了该年龄段正常及AIS椎弓根螺钉固定中进钉点、角度、螺钉直径及进钉深度等相关临床数据,部分(个性化螺钉置钉参数及导航模板3D打印)已在临床中应用,并取得良好的疗效;为该年龄段正常及AIS椎弓根螺钉固定的技术参数,最佳方案,内植物(螺钉及人工椎间盘)的设计积累了相关基础测量数据,同时也为矫形仪器的设计及AIS的诊治提供理论依据。课题的研究较为规范,严谨,基本按照原定计划完成,发表学术论文 9 篇,资助4名研究生开题及4项我校大学生英才培育项目完成,其成果参编出版4 部专著,9 部教材与图谱,获得省级科技进步二等奖(第2)和教学成果一等奖(第3)各1项,协助成功获批省级实验教学示范中心和数字转化医学工程技术研究中心,相关学术成果在第八届亚太解剖学国际大会,中国解剖学会年会,中华医学会数字医学分会年会上报告交流。
