New intracardiac ultrasound catheter with the function of tissue Doppler imaging and quantitative image analyzing technique were used to intensively study the temporal phase correlation between the cardiac electrical activation and myocardial mechanical response. It was the first time in the world to observe the structure of sino-atrial node(SAN) at real time and in vivo, and characterize the mechanical excitation over SAN area. Firstly, an image analyzing method to extract tissue acceleration, velocity and power information form tissue Doppler image was developed. Secondly, by analyzing the tissue Doppler image sequence of the tissue over SAN, acceleration-time curve(ATC) and synchronously recorded ECG curve, and PDF curve at typical cardiac phase of SAN were obtained. Thirdly, the temporal correlation between ECG and ATC was analyzed, and periodical and similar mechanical activation pattern was found. Finally, the spatiotemporal correlation between controlled electrical activation and myocardial mechanical response of normal and infarcted myocardium were studied, which will provide a new information for the diagnosis and WPW syndrome and ventricular arrhythmia.
采用新型超声探头进行组织多普勒成象,在国际上首先提出用外来电刺激心室壁不同部位,研究人工控制条件下电刺激及由此引起的心肌电兴奋和收缩活动之间的空间和时间关系及传播规律,经图象处理和多维重建,标测正常和缺血心肌中异位起搏点,为室性心律失常和预激综合症等心血管疾病的临床诊断和治疗提供依据,具有重要科学价值和实际社会经济效益。
